Study On The Correlation Between Different TCM Syndrome Types And Psychological Symptoms And Immune Factors In Recurrent Abortion Patients

Objective:This article investigates the general clinical data,TCM syndrome types,cellular immune function,and psychological symptoms of patients with recurrent abortion,and summarize the distribution pattern of TCM syndromes in patients with recurrent abortion.The relevant immune factors and different mental health conditions of patients with recurrent abortion are preliminarily discussed,in order to provide a scientific basis for the targeted improvement of the reproductive prognosis of women with recurrent abortion.Methods:In this study,a total of 122 patients with recurrent abortion who met the inclusion criteria were selected from the outpatient department of the First Affiliated Hospital of Shandong University of Traditional Chinese Medicine from December 2019 to December 2020.The included patients with recurrent abortion were dialectically classified according to the dialectical standards of traditional Chinese medicine,immunological related indicators were detected,and the general condition and mental health status of the patients were investigated by questionnaire.Excel software was used to establish the database,and spss24.0 statistical software was used for statistical processing.All data are tested for compliance with normal distribution.The measurement data conforming to normal distribution are expressed as the mean±standard deviation(x±s).Mean rank sum,quartile and median were used to represent those that did not conform to the normal distribution,and rank sum test was used.The correlation between syndrome types and immunological indicators and psychological symptoms in each group was analyzed.The results showed that P<0.05 was considered statistically significant.Results:1.The distribution of TCM syndromes of 122 patients with recurrent abortion:56 cases(46%)of liver stagnation and blood stasis type were the most,followed by 36 cases(30%)of kidney deficiency and blood stasis(30%),and 30 cases of spleen and kidney deficiency type(24%).The distribution of syndrome types had no correlation with age,number of abortions,,age of menarche,WHR and BMI(P>0.05).2.The level of tumor necrosis factor-α with recurrent abortion patients of liver stagnation and blood stasis was higher than that of kidney deficiency and blood stasis type,spleen and kidney deficiency type,and the difference was statistically significant(P<0.05).The level of anti-β2 glycoprotein IgG in patients with recurrent abortion of spleen and kidney deficiency was higher than that of liver stagnation and blood stasis,and the difference was statistically significant(P<0.05);There was no statistical difference among the three groups in lymphocyte subsets,lupus anticoagulant,and anticardiolipin antibody IgG grops(P>0.05).3.Among the 10 psychological dimensions of the SCL-90 scale,according to the factor scores of different TCM syndrome types,the highest score of kidney deficiency and blood stasis type was interpersonal sensitivity,followed by obsessive symptoms,and the lowest score was terror.The highest score of the spleen-kidney deficiency type was interpersonal sensitivity,followed by hostility,and the lowest was terror.The highest score of liver stagnation and blood stasis type was depression,followed by hostility,and the lowest score was terror.In addition,the scores of depression factors in the SCL-90 scale of RSA patients with liver stagnation and blood stasis were higher than those of RSA patients with spleen and kidney deficiency and blood,and the difference was statistically significant(P<0.05).Conclusion:1.The most common type of TCM dialectical classification of recurrent abortion patients was liver stagnation and blood stasis type followed by kidney deficiency and blood stasis type.2.The level of TNF-α and A-β 2GP1 in patients with recurrent abortion of liver stagnation and blood stasis type and spleen-kidney deficiency type were significantly higher than those of other syndrome types.3.The detection rate of psychological disorders in patients with recurrent abortion is relatively high,mainly manifested as depression,interpersonal sensitivity,hostility and compulsion.Depression factors in mental disorders are significantly related to liver stagnation and blood stasis type in TCM syndrome type.Patients with recurrent abortion of liver stagnation and blood stasis among the three TCM syndrome types should strengthen psychological intervention and counseling as well as clinical drug treatment.
