| Objectives:To explore the characteristics of five types of risk behaviors in common mental disorders:self-injury,suicide,wounding,destruction,and leave the army without permission behaviors,analyze the influencing factors of risk behaviors,and construct safety management countermeasures.Methods:(1)From October to December 2019,two psychiatrists in the final appraisal institution for mental disorders of recruits were recruited using the self-made"Recruit Common Mental Disorder Case Information Form"and"Recruit Common Mental Disorder Risk Behavior Interview Form"216 recruits diagnosed by physicians attending and above based on the International Classification of Diseases Tenth Edition(ICD-10)as adjustment disorder,dissociative disorders,mood disorder,and schizophrenia were investigated and interviewed using descriptive analysis,correlation analysis,chi-square test and other statistical methods are used to explore the characteristics of risk behaviors of common mental disorders in recruits.(2)From October to December 2019,the"Recruit Common Mental Disorder Case Information Form""Trait Coping Style Questionnaire""Military Social Support Scale"and"Recruit Training Period Stress Event Scale"were used for the above 216 recruits were investigated,and statistical methods such as descriptive analysis,chi-square test,Fisher’s exact probability test,Mann-Whitney U rank sum test,and binary logistic regression were used to analyze the influencing factors of common mental disorder risk behaviors in recruits.(3)Based on the above-mentioned research,explore the recruit’s risk behavior safety management countermeasures from the individual’s individual,family society,army,and state levels.Results:(1)The incidence of risk behaviors of common mental disorders among recruits was 42.6%,with the highest incidence of self-injury(34.3%),followed by suicide(13.9%),destruction(8.3%),wounded people(6.9%),leaving the post(5.6%).The incidence of self-injury behaviors of patients with different mental disorders is different(χ~2=8.087,P<0.05).The incidence of self-injury behaviors in patients with adaptation disorders and mood disorders is higher than that in patients with dissociative disorders(all P<0.05).The recurrence rate of dangerous behaviors common to recruits is 30%.The incidence of dangerous behavior aggregation in patients with mental disorders was 16.7%.(2)The common mental disorder dangerous behaviors of recruits are mainly hand cutting(33.7%),hitting a wall(20.7%),beating others with fists(15.2%),destroying personal belongings(14.1%),jumping from a building(13.0%),hammer wall/floor(12.0%),hammer head/arm/face(12.0%),and leaving the team without permission(10.9%).(3)Common causes for risk behaviors commonly encountered by recruits are escape from pain(65.2%),despair(65.2%),perceived burden on others(50.0%),guilt(35.9%),shame(26.1%),feeling depressed(12.0%),venting emotions(10.9%),interpersonal conflicts(10.9%).(4)common precursors of risk behaviors commonly encountered by recruits include tools to prepare for dangerous behaviors(23.9%),meaningless speech feeling or losing sense of purpose(21.7%),revealing thoughts about dangerous behavior(15.2%),restlessness(13.0%),gifting property and arranging inheritance,and other signals of being ready for death(9.8%).(5)It is relatively high for recruits to think of suicidal behaviors common to mental disorders for a period of time(63.3%),self-injury(73.0%),injury(93.3%),destruction(100%),leaving behavior(58.3%)accounted for impulse relatively high.(6)Injuries of common mental disorders among recruits occurred more frequently in the first month after enlistment(60.0%),among which suicide(56.7%),self-injury(44.6%),destructive behavior(44.4%),leave the army without permission behaviors(41.7%)occurred more frequently only in the second month after enlistment.(7)Consequences of risk behaviors commonly encountered by recruits include bleeding and scarring(43.5%),bruises and swelling(32.6%),damage to items(19.6%),and injuries to the victim(16.3%).(8)A history of risk behavior(OR=3.247,95%CI=1.740~6.06,P<0.05),easy to be pessimistic(OR=2.031,95%CI=1.090~3.785,P<0.05),negative coping before enlistment(OR=1.060,95%CI=1.011~1.112,P<0.05)is a risk factor for dangerous behaviors of common mental disorders in recruits,and the utilization of social support is high(OR=0.813,95%CI=0.671~0.986,P<0.05)is a protective factor for its dangerous behavior.Conclusions:(1)Self-injury and suicide are more common dangerous behaviors for recruits with common mental disorders.(2)The incidences,methods,causes,precursors,thinking times,and occurrence times of different dangerous behaviors are different.(3)A history of dangerous behavior,easy pessimism,and negative coping are common risk factors for risk behaviors in recruits before they join the army,and high utilization of social support is a protective factor for their risk behaviors.(4)The safety management of recruits’dangerous behaviors can start with recruits self-adjustment,family care support,social support for soldiers,dangerous goods control,emergency crisis intervention,mental health screening tools,and professional structure. |
