| Objective To observe the frequency of IL-17 and Foxp3 in the peripheral blood and the expression level of PD-1/PD-L1 on the surface of CD4+ T cells in patients with chronic hepatitis B(CHB),which treated by Yanggan Yishen method combined with entecavir(ETV).Observe the impact of ALT normalization rate,hepatitis B virus Deoxyribonucleic acid(HBV DNA)negative conversion rate and hepatitis B e-antigen(HBe Ag)serological conversion rate,and liver fibrosis in patients with CHB Items,FIB-4 index.Methods Sixty patients with CHB who met the criteria were randomly divided into 30 cases,which incloud treatment group and control group.The treatment group was treated with Yanggan Yishen method combined with ETV,and the control group was treated with ETV alone.The treatment course was 48 weeks.The frequency of IL-17 and Foxp3 in the peripheral blood and the expression level of PD-1/PD-L1 on the surface of CD4+ T cells were measured before treatment(0 week),24 weeks and 48 weeks after treatment,and blood routine test,liver function,HBV DNA,hepatitis B three line,and four liver fibrosis.Results After 48 weeks of treatment,the frequency of Th17 and Foxp3 in treatment group decreased significantly compared with that before treatment(P<0.05),and the difference was statistically significant(P<0.05).PD-1/PD-L1’s expression levels decreased significantly compared with the control group,there was a statistically significant difference(P <0.05).The expression of PD-1/PD-L1 on CD4+ T cells in both groups at 24 and 48 weeks after treatment,which were significantly lower than those at baseline(P<0.05).After 48 weeks of treatment,the negative rate of HBV DNA and the rate of abnormal ALT in treatment group were significantly higher than those in the control group(P<0.05).HBe Ag seroconversion rate was no significant difference between the two groups(P>0.05).After 48 weeks of treatment,two groups of four liver fibrosis indicators decreased than before,the treatment group HA and LN were lower than the control group,and the differences between groups were statistically significant(P<0.05),and the treatment group PC III and IV-C is lower than that of control group,but there were no significant differences between the groups(P>0.05);after 48 weeks of treatment,patients with FIB-4 index of two groups were decreased,the FIB-4 index of treatment group were lower than the control group,and the differences between groups were statistically significant(P<0.05).Conclusion Yanggan Yishen method can increase the negative rate of HBV DNA and ALT recurrence rate,but it has no obvious effect on HBe Ag seroconversion rate.Yanggan Yishen method can regulate the immune function of patients with CHB,enhance the antiviral efficacy,and have a certain anti-hepatic fibrosis effect. |
