| This paper is divided into two parts.The first part is literature review.The review of traditional Chinese medicine summarizes the understanding,diagnosis and treatment progress of traditional Chinese medicine on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)from four aspects:overview,etiology and pathogenesis,syndrome classification and treatment.The review of Western medicine summarizes the progress of modern medicine in the acute exacerbation of chronic obstructive pulmonary disease from the aspects of overview,etiology and inducing factors,pathogenesis,diagnosis and evaluation,treatment and so on.The second part is clinical research.Objective:1 The clinical efficacy of rhubarb acupoint application at Shenque point in the adjuvant treatment of AECOPD with Tanrefushi;2.Explore the mechanism of its action;3.Evaluate safety.Methods:From March 2019 to March 2020,60 patients with AECOPD Tanrefushi syndrome in the respiratory ward of Tongzhou Hospital of Dongzhimen Hospital of Beijing University of traditional Chinese medicine were randomly divided into two groups(control group and experimental group).The control group was treated with Western medicine combined with traditional Chinese medicine decoction Maxing Shigan Decoction Combined with Sangbaipi Decoction,On the basis of the control group,the experimental group applied rhubarb acupoint to Shenque acupoint,recorded the general information of the patients,and recorded the total score of symptoms,the score of cough,expectoration,wheezing and other symptoms,MMRC score,cat score,leukocyte count,percentage of neutrophils and C-reactive protein on the first,fifth and tenth day of admission.Establish database,input statistical software and conduct data statistical analysis.Results:A total of 60 cases were collected clinically,including 4 cases of abscission and 56 cases with complete data,including 28 cases in the experimental group and 28 cases in the control group.1.Before treatment,the general data of the two groups were compared,and the demographic data(gender,age,course of disease,severity evaluation)had no statistical difference(P>0.05).2.Before treatment,there was no significant difference between the two groups in total symptom score,cough,expectoration,wheezing and other symptom scores,MMRC score,cat score and serological indexes(leukocyte count,neutrophil percentage,CRP)(P>0.05).3.Analysis of curative effect:in the experimental group,2 cases were clinically controlled,6 cases were markedly effective,19 cases were effective and 1 case was ineffective,with a total effective rate of 96.4%.In the control group,2 cases were clinically controlled,4 cases were markedly effective,18 cases were effective and 4 cases were ineffective,with a total effective rate of 85.7%,There was no significant difference between the two groups(P>0.05)4.At the observation node on the 10th day,there was significant difference between the two groups in total symptom score,abdominal distention,constipation and cat score(P<0.05),indicating that the test group was better than the control group in total symptom score,abdominal distention,constipation and cat score.5.There was no significant difference in cough,expectoration,wheezing,MMRC score,WBC,NE%and CRP between the two groups(P>0.05)6.Before and after treatment,the liver function(ALT)and renal function(crea)of the two groups were within the normal range,indicating that the application of rhubarb acupoint at Shenque point combined with the treatment scheme of integrated traditional Chinese and Western medicine is safe and reliable.Conclusion:There was no significant difference in the total efficacy of rhubarb acupoint application at Shenque point combined with western medicine+traditional Chinese medicine decoction and Western medicine+traditional Chinese medicine decoction in the treatment of AECOPD patients with phlegm heat Fu syndrome;There was no significant difference between the two groups in the improvement of cough,expectoration and wheezing symptoms,MMRC score,WBC,NE%and CRP.The symptoms of abdominal distension and constipation were improved,and there was significant difference in cat score between the two groups.The combined rhubarb acupoint application group was better than the control group,and the optimal time point was the 10th day.There were no adverse reactions in both groups,and the short-term application was safe and reliable... |
