| Objective:This study is to sort out the knowledge of etiology and pathogenesis of stroke,clinical experience and commonly used prescriptions for treating stroke,summarize the characteristics of its quasi-prescription,explore the law of medication,and provide some empirical reference and evidence-based basis for clinical treatment and prevention of stroke in the future.Material and method:1.Retrieval method of TCM philology research:The 5th edition of Chinese Medical Code gathers 1,156 TCM works from previous dynasties before the founding of New China,which are used as the retrieval scope and main tools of TCM literature,with stroke,stroke,hemiplegia,paralysis,thin fainting,big fainting,partial withering,and weak fainting.2.St atistical analysis:Based on the collected ancient books and literature,the effective prescriptions of "exogenous wind" theory are screened out,and the corresponding database is established by Excel on windows 10 system.On this basis,the origin,age,frequency,efficacy,four flavors and five flavors of the effective prescriptions are analyzed by using the software of "Chinese Medicine Inheritance Assistant Platform",and the new prescription is simulated by using the calculation function of the software.3.Theoretical exploration:combining the contents of ancient literature with the theory of traditional Chinese medicine to study and analyze the data results,summarizing the understanding of ancient physicians on the pathogenesis of stroke,the basis of dialectical treatment under the influence of the theory of "exogenous wind" and the law of syndrome and treatment of prescription medication.Results:1.A total of 189 effective prescriptions of exogenous theory have been searched in the literatures that meet the criteria of inclusion and exclusion,including 2 effective prescriptions of Han Dynasty,2 effective prescriptions of Jin Dynasty,2 effective prescriptions of Southern and Northern Dynasties,23 effective prescriptions of Tang Dynasty and 160 effective prescriptions of Song Dynasty.Medical works in Tang and Song Dynasties accounted for the largest proportion,with the total accounting for more than 90%,while other times were less.2.Frequency statistical results:Among the 189 effective prescriptions with exogenous wind theory,225 kinds of drugs were involved,and the total frequency of drug use reached 2697 times,among which the top 10 drug use frequencies were Ephedra(128),Saposhnikovia.divaricata(123),Chuanxiong(114),Guizhi(108).Aconitum carmichaeli(96),Glycyrrhiza uralen sis Fisch.(88),etc.In Fang Yaozhong,the proportion is Ephedra sinica(67.7%),Saposhnikovia divaricata(65.1%),Chuanxiong(60.3%),Cinnamomum cassia twig(57.1%),Aconitum carmichaeli(50.8%).Glycyrrhiza uralensis Fisch(46.6%),Angelica sinensis(42.3%),Ginseng(40.2%)3.Analysis results of four qi and five flavors of drugs:225 kinds of drugs are involved in 189 prescriptions which are effective in the theory of exogenous wind.Among the five flavors of drugs,pungent drugs account for the most,followed by bitter drugs and sweet drugs,while salty drugs,sour drugs and astringent drugs account for the least,with the proportion being pungent(40%),bitter(28%),sweet(26%)and salty(26%)Among the four qi drugs,warm drugs account for the most,followed by cold drugs and flat drugs,while hot drugs and cool drugs account for the least,with the proportion being warm(54%),cold(19%),flat(17%),hot(7%)and cool(3%)in turn.4.High-frequency drug association analysis:the number of support:40;Under the condition of confidence:0.6,the drug combinations with frequency greater than 70 are:Ephedra-Chuanxiong,Ephedra-Saposhnikovia divaricata,Ephedra-Ramulus Cinnamomi,Ramulus Cinnamomi-Saposhnikovia divaricata,Chuanxiong-Ramulus Cinnamomi,Ephedra-Chuanxiong-Saposhnikovia divaricata,Ephedra-Glycyrrhiza,Ephedra-Chuanxiong-Ramulus Cinnamomi,Ephedra-Ramulus Cinnamomi-Saposhnikovia divaricata.The frequency of occurrence is 99,97,88,86,84,79,75,71,71 and 71.5.New simul ation analysis:in the correlation degree:5;Under the condition of punishment degree:2,27 groups of new prescription combinations were obtained by clustering calculation with the software of "Traditional Chinese Medicine Inheritance Auxiliary Platform",among which the representative new prescription combinations are:Schisandra chinensis-dried ginger-Tianxiong,Schisandra chinensis-Evodia rutaecarpa-Tianxiong,Ophiopogon japonicus-Rehmannia glutinosa-Salvia miltiorrhiza,Almond-Chuanxiong-Gypsum,Rehmannia glutinosa-D endrobium-Salvia miltiorrhiza and Scorpio-Zaocys Ther e are 32 groups of drug combinations with correlation coefficient greater than 0.05,and the drug pairs with high correlation are:scorpion-Arisaema,musk-Bombyx batryticatus,scorpion-Aglcistrodon,scorpion-bezoar,zaocys-Gastrodia elata,musk-Gastrodia elata,Bombyx batryticatus-cinnabar,zaocys-bezoar,bezoar-Gastrodia elata,scorpion-cicada,musk-bezoar.Conclusion:1.Most of the effective prescriptions of "exogenous wind" theory come from medical works in Tang and Song Dynasties.2.The effective prescriptions of the theory of "external wind" are most widely used in the selection of drug efficacy,such as exterior-relieving drugs,blood-activating and blood-stasis-removing drugs,rheumatism-dispelling drugs,liver-calming and wind-calming drugs,etc.,which is in line with the understanding that "internal deficiency and evil middle"was the main pathogenesis of stroke before Tang and Song Dynasties;3.Warm drugs and pungent drugs account for the most in the choice of drug taste in the effective prescriptions of "exogenous wind" theory;4.The combination of Radix Ophiopogonis-Radix Rehmanniae Preparata-Radix Salviae Miltiorrhizae,Radix Rehmanniae Preparata-Herba Dendrobii-Radix Salviae Miltiorrhizae is the most significant among the new combinations calculated by clustering the drugs in the effective prescriptions of "exogenous wind" theory. |
