Purpose : In this study,the SD rats were pretreated with Xijing Tongmai Decoction combined with acupuncture at Neiguan point,Xinshu point and Zusanli point,and the rats were fed with a high-fat diet combined with subcutaneous injection of isoproterenol hydrochloride to induce acute myocardial infarction.To study the regulatory effects of acupuncture and drug combination preconditioning on electrocardiogram,myocardial histomathology and serum CTRP3,ADMA and hs-CRP in rats with acute myocardial infarction,and to explore the potential protective effect and possible mechanism of acupuncture and drug combination preconditioning on rats with acute myocardial infarction.Material and method : According to random number table method,50 rats were randomly divided into 5 groups: control group,model group,acupuncture and drug combination pretreatment group,traditional Chinese medicine pretreatment group,and western medicine pretreatment group,with 10 rats in each group.Acupuncture and medicine combination pretreatment group: at 8 o ’clock a day,the group was given 19.35g/(kg·d)Jiejingtongmai decoction by intragastric administration,once a day,for 14 days.Electroacupuncture intervention was performed at 10 o ’clock every day.Bilateral "Neiguan",bilateral "Xinshu" and bilateral "Zusanli" were selected,and the needles were kept for 15 min,once a day,for 14 days.The specific acupuncture methods are as follows:(1)The rats were prostrated on the operating table and bound and fixed.(2)Referring to the positioning method of acupoints of rats and acupoints in Experimental Acupuncture and Moxibustion,Neiguan acupoint is selected on the inner side of the forelimb,saving about3 mm between the ulna and radius from the wrist,and 1 acupoint on the left and right sides;Xinshu acupoint was selected on the back of the rat,located on both sides of the fifth thoracic vertebra,between the flat ribs,with 1 acupoint on the left and right sides;Zusanli acupoint is selected on the posterolateral side of the knee joint,about 5mm below the fibula head,and 1 point about each.After setting the acupoints,conventional local disinfection was carried out,and acupuncture was performed with Huatuo brand disposable sterile acupuncture needles(0.18*25mm).(3)Neiguan point straight stab 0.2-0.3cm;Xinshu point inward oblique thorn 0.2-0.3cm;Zusanli point straight stab 0.2-0.3cm.After the rat was stabilized,the positive and negative poles of the same wire were connected to the ipsilateral Neiguan and Zusanli points.(4)Density wave,frequency 2-20 Hz,current 2-3m A,the intensity of current is appropriate to the presence of slight tremor in the forelimbs and hind limbs consistent with the frequency of electroacupuncture.In the traditional Chinese medicine pretreatment group,19.35g/(kg·d)Xijing Tongmai Decoction was administered intragastrically at 8 o ’clock every day,which was equally bound and fixed without acupuncture,once a day,for 14 days.Western medicine pretreatment group: Aspirin enteric-coated tablet 9mg/(kg·d)was given by gavage at 8 o ’clock a day,and fixed by equal binding without acupuncture,once a day,for 14 days.Model group: at 8 o ’clock a day,the same amount of normal saline was given to the stomach,the same binding was fixed,and the rest were fed normally for 14 days without any intervention factors.The control group was given the same amount of normal saline by gavage at 8 o ’clock a day,and was equally bound and fixed.The rest were fed normally for 14 days without any intervention factors.During the intervention period,rats in each group were fed with normal diet and fed water freely,and the general state of rats was observed continuously.After 14 days of pretreatment,the acupuncture and drug combination pretreatment group,Chinese medicine pretreatment group,western medicine pretreatment group and model group were modeled.Forty rats in model group were fed with high-fat diet15g/ rat for 21 days after the normal diet was stopped.10 rats in control group were fed the same amount of normal diet.The pretreatment intervention was carried out for 21 days as before.22 days 10 when all rats by intraperitoneal injection of the left lower quadrant abdominal 10% chloral hydrate anesthesia,and supine position fixed on the operating table,40 rats only build module based on ISO suspension point injection using disposable 85 mg/kg copying animal model,10 control group rats with limbs root subcutaneous one-time multi-point injection volume such as physiological saline,The second injection was conducted at an average interval of 24 h.The Power Lab data acquisition and analysis system was used to record the Ⅱ lead electrocardiogram.According to the change of T wave and ST segment of electrocardiogram of rats before and after modeling,the success of modeling was determined.The myocardial histopathology,serum levels of CTRP3,ADMA and hs-CRP were detected 24 hours after successful modeling.Results:1 Life state observation of rats in each group During the adaptive feeding stage,the diet and excretion of rats in each group were normal,their mental state was good,their hair color was bright,their movements were quick,and their body weight increased normally.During the pretreatment stage,there was no significant change in the life state of rats in each group.In the modeling stage,there was no significant change in the control group.The weight gain of rats fed the high-fat diet was significantly higher than that of the control group.The hair color was slightly yellow,the gloss decreased,and there were different degrees of listlessness and retarded reaction.After modeling,there was no significant change in the control group,while the other groups showed different degrees of dry and dull hair,listlessness of spirit,reduced activity,bluish lips,tongue and paw armour,with the most serious in the model group and the least in the acupuncture and drug combination pretreatment group.2.Comparison of voltage changes in ST segment of ECG of rats in each group showed that:After modeling,compared with the control group,the changes of electrocardiogram ST segment voltage in model group,acupuncture and medicine combination pretreatment group,traditional Chinese medicine pretreatment group and western medicine pretreatment group were statistically significant(P < 0.01).Compared with model group,the changes of electrocardiogram ST segment voltage in acupuncture and drug combination preconditioning group,Chinese medicine pretreatment group and western medicine pretreatment group were statistically significant(P < 0.01),and the changes of electrocardiogram ST segment voltage in acupuncture and drug combination preconditioning group were better than those in Chinese medicine and western medicine pretreatment group(P < 0.05).There was no significant difference in ST segment voltage between TCM pretreatment group and western medicine pretreatment group.3.Pathological changes of left ventricular myocardial tissue in each group(microscope 200X)By observing the HE staining section of the myocardial tissue of rats in each group,it was found that in the control group,the structure of myocardial cells was regular,the arrangement of myocardial fibers was regular,the disc and striated muscle were clear,and there was no fracture,defect or blank area.Model group: myocardial cell structure was obviously disordered,myocardial fibers were arranged irregularly and accompanied by swelling,a large number of inflammatory cells were infiltrated,disc running and striated muscles were blurred,and obvious fractures,defects and blank areas were visible.In the acupuncture and drug combination preconditioning group,compared with the model group,the structure of myocardial cells was basically normal,the arrangement of myocardial fibers was basically orderly,the disc and striated muscle were basically normal,the infiltration of inflammatory cells was significantly reduced,and occasionally slight fractures,defects and blank areas were observed.Compared with the model group,the structure of myocardial cells was improved,the arrangement of myocardial fibers was more orderly,the disc and rhabdomyin were normal,the infiltration of inflammatory cells was significantly reduced,and occasionally slight fractures,defects and blank areas were observed in some parts.In the western medicine preconditioning group,compared with the model group,the structure of cardiac muscle cells was relatively regular,the arrangement of cardiac fiber was repaired,the disc and striated muscle were restored,the inflammatory cells were slightly reduced,and there were fractures,defects and blank areas in some parts.4.Comparison of serum CTRP3 levels in each group Compared with the control group,the level of CTRP3 in the model group,the acupuncture and medicine combination pretreatment group,the traditional Chinese medicine pretreatment group and the western medicine pretreatment group decreased significantly(P <0.01).Compared with model group,the level of CTRP3 in acupuncture and medicine combination preconditioning group,traditional Chinese medicine preconditioning group and western medicine preconditioning group increased significantly(P < 0.01).Compared with the traditional Chinese medicine pretreatment group,the level of CTRP3 in the acupuncture and medicine combination pretreatment group and the western medicine pretreatment group was significantly higher(P < 0.05).There was no significant difference in CTRP3 level between the acupuncture and medicine combination pretreatment group and the western medicine pretreatment group(P > 0.05).5 Comparison of serum ADMA levels in each group Compared with the control group,ADMA levels in model group,acupuncture and drug combination pretreatment group,Chinese medicine pretreatment group and western medicine pretreatment group were increased,with significant differences(P < 0.01).Compared with model group,the ADMA level of acupuncture and medicine combination pretreatment group,traditional Chinese medicine pretreatment group and western medicine pretreatment group decreased significantly(P < 0.01);Compared with the acupuncture and medicine combination pretreatment group,the ADMA level of TCM pretreatment group and western medicine pretreatment group decreased significantly(P < 0.05).There was no significant difference in ADMA level between TCM pretreatment group and western medicine pretreatment group(P>0.05).6.Comparison of serum hs-CRP levels in each group Compared with the control group,the level of hs-CRP in the model group,the traditional Chinese medicine pretreatment group and the western medicine pretreatment group increased significantly(P < 0.01).Compared with model group,the level of hs-CRP in acupuncture and medicine combination pretreatment group,traditional Chinese medicine pretreatment group and western medicine pretreatment group decreased significantly(P < 0.01).Compared with the acupuncture and medicine combination pretreatment group,the hs-CRP level in the traditional Chinese medicine pretreatment group and the western medicine pretreatment group was significantly higher(P < 0.01).Compared with the traditional Chinese medicine pretreatment group,the content of hs-CRP in the western medicine pretreatment group decreased significantly(P < 0.05).Compared with the control group,there was no significant difference in hs-CRP content between the acupuncture and drug combination pretreatment group(P > 0.05).Conclusion:1.The preconditioning of SD rats with the combination of Zhidingtongmai decoction and acupuncture at Neiguan,Xinshu and Zusanli points has a potential preventive effect on the rat model of acute myocardial infarction,and can significantly reduce the pathological changes of myocardial tissue in the rat model of acute myocardial infarction.It has a benign regulation effect on ECG,serum CTRP3,ADMA and hs-CRP levels in rats with acute myocardial infarction.2.Compared with traditional Chinese medicine preconditioning and western medicine preconditioning,acupuncture combined with medicine preconditioning has significant advantages in the prevention of acute myocardial infarction in rats.
