| Objective:To explore the distribution characteristics of TCM syndromes and risk factors in patients with colorectal polyps,so as to provide clinical basis for reducing the recurrence and canceration of the disease.Methods:This study was based on the study of TCM syndromes and risk factors of colorectal polyps in basic scientific research(young teachers project)of Beijing University of traditional Chinese medicine in 2017.Data of 160 patients and 160 control groups were analyzed and mined.(1)For the screening of related factors in patients with colorectal polyps,chi square test and nonparametric test of two independent samples were used,and the factors with significant differences between the two groups were involved in logistic regression analysis to obtain the risk factors of colorectal polyps.(2)With the help of principal component factor analysis,the distribution characteristics of TCM syndromes in patients with colorectal polyps were studied.(3)The location and nature of colorectal polyps were compared by frequency.Results:(1)Distribution of clinical data in normal group:160 healthy subjects were included in normal control group.Gender:74 male(46.25%),86 female(53.75%),median age:48(36-57.75),lower quartile:36,upper quartile:57.75,median:48.(2)The prevalence of colorectal polyps and related factors:160 cases of colorectal polyps patients,gender:109 cases of male(68.13%),51 cases of female(31.88%),median age 59(50-65)years old,(lower quartile 50,upper quartile 65,median 59).There were 35 cases(32.11%)of male patients aged 61-70 and 33 cases(30.28%)of male patients aged 51-60.There were 19(37.25%)female patients aged 61-70 years and 17(33.33%)female patients aged 51-60 years.A total of 160 patients with colorectal polyps were included in the study.Binary logistic regression analysis showed that there were 9 risk factors for the onset of colorectal polyps,including age(P=0.003),family history of colorectal polyps(P=0.009),HP(positive)(P=0.029),smoking(P<0.001),high-fat diet preference(P=0.037),high salt and pickled food diet preference(P=0.014),smoking(P<0.001)Oral non steroidal anti-inflammatory drugs(P<0.001),oral antibiotics(P<0.001),mood(P<0.001)and hypertension(P=0.02)were the risk factors of colorectal polyposis.(3)Using principal component factor analysis to explore the distribution characteristics of TCM syndromes:160 cases of colorectal polyps were analyzed by frequency analysis.The common TCM symptoms were irritability(63.10%),short yellow urine(61.30%),abnormal taste(56.90%),dry mouth(52.50%),chest fullness(50.60%),loose stool(50.00%),irritability(41.90%),fatigue(41.30%),heave a deep sigh(40.00%),bowel sounds and exhaust(39.40%),abdominal distension(37.50%),abdominal tingling(36.30%),chilly limbs(22.50%),etc.In the aspect of tongue appearance:tooth marks(79.40%),red tongue(52.50%),light red tongue(48.10%),white fur(39.40%),yellow fur(33.80%),thin fur(45.00%),greasy fur(39.40%),thick fur(38.80%),and fat tongue(25.00%).Pulse condition:slippery pulse(49.40%),string pulse(37.50%)and deep pulse(31.30%).Principal component factor analysis showed that there were 8 common syndromes in patients with colorectal polyps:Damp heat syndrome(31.25%)was the most common,blood stasis syndrome(11.25%),spleen qi deficiency and dampness accumulation syndrome(10.63%),disharmony between liver and stomach syndrome(10.63%),spleen and stomach weakness syndrome(10.63%),liver depression and spleen deficiency syndrome(10.00%),liver depression and fire syndrome(8.13%),cold pathogen syndrome(7.50%).The main disease nature of syndrome elements of colorectal polyps were:dampness(26.17%),heat(24.60%),Qi deficiency(19.53%),Qi Stagnation(17.97%),blood stasis(7.03%)and cold(4.69%).The main locations of disease in colorectal polyps were large intestine(42.70%),spleen(22.00%),liver(20.30%),stomach(15.00%)(4)The location of colorectal polyps:sigmoid colon(27.70%),rectum(20.40%),transverse colon(13.10%),ascending colon(11.80%),descending colon(10.00%),colon liver(9.70%),ileocecal(4.80%),colon spleen(2.40%)(5)Analysis of the nature of polyps:the pathological report of patients with tubular adenoma polyps(78.13%),villous tubular adenomas(18.75%),chronic inflammatory polyps(3.13%)Conclusion:(1)there are 9 risk factors for colorectal polyps,which are age,family history of colorectal polyps,HPinfection,smoking,diet preference,oral non steroidal anti-inflammatory drugs and antibiotics,mood and hypertension.(2)The four diagnostic signs of traditional Chinese medicine are mainly dampness,heat(fire),Qi deficiency,qi stagnation,blood stasis and cold coagulation.The main syndrome types were 8.(3)Damp heat in large intestine,stagnation of blood stasis,deficiency of spleen qi and accumulation of dampness in large intestine,disharmony between liver and stomach,weakness of spleen and stomach,stagnation of liver qi and deficiency of spleen,transformation of liver Qi into fire,internal obstruction of cold pathogen.The syndrome elements were dampness,heat(fire),Qi deficiency,qi stagnation,blood stasis and cold coagulation.The location of the disease was large intestine,spleen,stomach and liver.(3)Location and nature of colorectal polyps:multiple in sigmoid colon,followed by rectum.Tubular adenoma was the most common pathological report. |
