Observation Of The Curative Effect Of Lou Qin Zhisou Decoction In The Treatment Of Bronchiectasis Complicated By Infection With Phlegm-heat Obstructing The Lung Syndrome

This thesis is mainly divided into two parts.The first part is literature review:Literature review summarizes the research of bronchiectasis in recent years from two aspects of western medicine and traditional Chinese medicine.Western medicine review summarizes the research progress of bronchiectasis from the aspects of definition,epidemiology,etiology,pathogenesis,treatment and medication.The summary of Chinese medicine summarizes the understanding of bronchiectasis in ancient Chinese medicine from the following aspects:the name,etiology and pathogenesis,syndrome differentiation and related treatment methods of bronchiectasis.The second part is clinical research:Objective:To observe the improvement of scores of major and minor TCM syndromes,total TCM syndrome scores,curative effect observation of TCM syndromes and mMRC of outpatients with bronchiectasis complicated with infection,observe the changes of inflammatory indicators:blood routine,neutrophil percentage and C-reactive protein before and after treatment,observe the safety and adverse reactions of this study,and then evaluate the results.Methods:This study included patients who were diagnosed as bronchiectasis and complicated with infection with TCM syndrome of phlegm-heat obstructing lung from January 2020 to May 2020 in the respiratory outpatient department of Tongzhou Campus of Dongzhimen Hospital of Beijing University of Traditional Chinese Medicine.They were randomly divided into experimental group and control group.The experimental group was given oral Chinese medicine granule,Professor Wu Weiping’s experience prescription,Youqin Zhisoujian combined with western medicine antibiotic(levofloxacin).The observation time was 3 days.Collect the basic information,blood routine,neutrophil percentage,C-reactive protein,TCM syndrome and mMRC data of patients,establish a database,and analyze the data by SPSS 26.0 software.Results:According to the standard of admission and exclusion,a total of 60 patients with bronchiectasis complicated with infection were included,among which 1 case fell off(lost follow-up),1 case was eliminated(taking other drugs),and 1 case fell off in the control group(lost follow-up).Finally,there were 28 cases in the experimental group,and one more patient in the control group than in the experimental group.There were 27 males and 30 females.The youngest is 24 years old and the oldest is 75 years old.T he average age was 52.57±12.580 years in the experimental group and 53.66± 14.734 years in the control group.Before treatment,the general data of the two groups were compared.The demographic data(gender,age)of the two groups had no statistical difference(P>0.05),but were comparable.Serological comparison:before treatment,the data of leukocyte,neutrophil and CRP were compared,P>0.05,so the above serological indexes are comparable.Chinese medicine score scale(TCM syndrome integral and syndrome total integral)and mMRC score were compared before treatment,and there was no statistical difference(P>0.05),but there was comparability.Intra-group comparison:after treatment,the percentage of white blood cells,neutrophils and CRP in the experimental group were statistically significant(P<0.05),indicating significant curative effect.Tcm syndrome scores such as cough,expectoration,chest tightness,shortness of breath,fatigue,anorexia,abdominal distension,and the total score of syndrome,mMRC score and TCM syndrome curative effect comparison all indicated significant curative effect(P<0.05).The percentage of white blood cells and neutrophils in the control group was significantly different after treatment(P<0.05),and the treatment was effective,while the indexes of CRP did not improve significantly after treatment(P<0.05).In TCM syndrome score,the scores of cough,expectoration,chest tightness,shortness of breath,fatigue,total score of syndrome,mMRC score and TCM syndrome curative effect were statistically analyzed(P<0.05),so the above scores were improved.Comparison between groups:There was significant difference in neutrophil and CRP between the experimental group and the control group(P<0.05),indicating that the experimental group was more effective.Compared with the scores of cough,expectoration,chest tightness,shortness of breath,and anorexia,there is statistical significance(P<0.05),as well as the comparison of curative effect of TCM syndromes and the improvement of mMRC scores.However,there was no significant difference between the two groups in the improvement of leukocyte index and syndrome total score(P<0.05).The ALT,UREA,CREA indexes of the two groups before and after treatment were all within the normal range,which indicated that the Chinese medicine decoction,Luqin Zhisoujian,was safe and reliable.Conclusion:It is effective to treat bronchiectasis complicated with infection with phlegm-heat obstructing the lung on the basis of anti-infection by Western medicine alone and anti-infection treatment by Western medicine combined with traditional Chinese medicine.On the improvement of inflammatory indicators(leukocyte,neutrophil percentage,C-reactive protein),TCM syndrome integral,comparison of curative effect of syndrome integral and mMRC score,the curative effect of western medicine combined with Louqin Zhisoujian is obviously better than that of western medicine alone.It shows that Chinese medicine has great advantages in the clinical treatment of bronchiectasis complicated with infection,and has strong clinical promotion value.
