Objective:With the improvement of people’s living standard and the change of diet structure, the incidence rate of gallbladder related diseases is also increasing year by year.At present, laparoscopic cholecystectomy(LC)is the first choice for gallbladder benign diseases with operative indication because of its small wound and high acceptability compared with traditional open surgery. However, there will be a series of clinical symptoms of gastrointestinal dysfunction due to its inevitable traumatic operation, which will affect the postoperative recovery of patients. The traditional western medicine has a single conventional treatment method and high treatment cost. Therefore, this study attempts to use external treatment of traditional Chinese medicine, namely acupoint application combined with Tongfu Chengqi Decoction enema,to observe whether it can treat gastrointestinal dysfunction after LC operation,to help patients accelerate gastrointestinal function recovery after laparoscopic cholecystectomy, improve the score of gastrointestinal function after operation, and reduce postoperative pain,It provides a new treatment method for gastrointestinal dysfunction after laparoscopic cholecystectomy with Chinese medicine.Method:According to the inclusion and exclusion criteria,60 patients with laparoscopic cholecystectomy were randomly divided into treatment group and control group,30 cases in each group. On the basis of conventional treatment, the experimental group began to apply rhubarb and cinnamon at bilateral Hegu and Zusanli points 0.5 h after operation, once a day for 2 h;At the same time,Tongfu Chengqi Decoction retention enema treatment was started 8 hours after operation, one dose a day,400 ml water decoction,200 ml each time for retention enema, once a day in the morning and evening,30 minutes each time. The first defecation was the cut-off point of the above treatment, and the longest treatment time was 5 days.If there was no spontaneous defecation after 5 days, other treatment measures were taken. The clinical efficacy, bowel sounds recovery time, first exhaust and defecation time, abdominal drainage tube indwelling time, gastrointestinal function recovery score and postoperative VAS score were recorded.Result:1.Before treatment, the general information and operation related information of the two groups were compared, and the baseline was comparable.2.Compared with the control group, the first spontaneous exhaust defecation time and bowel sounds recovery time of the experimental group were shorter, the difference was statistically significant(P<0.05).3.The indwelling time of abdominal drainage tube in the experimental group was 39.70±15.30h,and that in the control group was 50.70+17.82h,the difference was statistically significant.4.The overall efficacy score of gastrointestinal function recovery at 12h,24h and 48h after operation in the experimental group was higher than that in the control group at the same time point(P<0.05).5.Compared with the control group at 12h,24h and 48h after operation,TCM external treatment can reduce the postoperative pain of LC patients, and the difference is statistically significant(P<0.05).6.The total effective rate of the experimental group was 96.7%,which was significantly higher than 73.3%of the control group(P<0.05).Conclusion:For patients after laparoscopic cholecystectomy, acupoint application and Tongfu Chengqi Decoction enema can be given on the basis of conventional treatment of Western medicine to promote the recovery of gastrointestinal function, reduce the pain of patients, shorten the treatment time, and its safety is good, which is worthy of clinical application.
