Objective Through the retrospective analysis of the clinical data of Kawasaki disease,the clinical manifestations and laboratory indexes of patients with and without coronary artery lesions were compared,the risk factors of coronary artery lesions were analyzed,and the clinical value of related indicators in Kawasaki Disease combining with coronary artery lesions were investigated,all of which to get treatment as early as to minimize the adverse consequences of complications.Methods A total of 157 patients admitted to the First Hospital of Lanzhou University from January 2017 to December 2021 were retrospectively enrolled in the study and divided into two groups:the CALs group and the n CALs group,and the differences of clinical and laboratory data between the two groups were compared.The risk factors of patients suffering from coronary artery lesions were analyzed by binary logistic regression,and the efficacy of related indicators in predicting coronary artery lesions was analyzed by receiver operating characteristic curve,and when P<0.05 was statistically significant.Results1)In this study,the incidence rate of Kawasaki disease in the hospital was the highest in summer,which of the peak was in August,while the incidence rate of the CALs group was the highest in spring.And in different months,the incidence rate of male children is generally higher than that of female children.2)Non-suppurative conjunctival hyperemia was the most common clinical manifestation in KD children(85.35%),scarlet fever-like rash or erythema multiforme rash was the second(75.80%),and cervical lymphadenopathy was the third(75.16%),but the occurrence of scar(5.73%)and metacarpal erythema(3.82%)is relatively rare.3)Compared with n CALs group,CRP、PCT、ESR、RDW、PLT,D-D and FDP were obviously increased in CALs group,whlie Hb、HCT、MCV、MCH、MCHC and Na+were obviously decreased in CALs group,and there were significant differences between CALs group and n CALs group in the clinical presentation of strawberry tongue and anemia(P<0.05).4)Binary logistic regression analysis showed that all of the level of PCT,RDW,PLT and D-D increased,the serum of Sodium decreased and the clinical presentation of strawberry tongue were risk factors for the development of CALs in KD.5)The ROC curve analysis showed that before IVIG and aspirin were used in KD children,the best cut-off value of RDW、PCT、D-D and PLT to predict what would happen to CALs in KD respectively were 13.35%、0.705ng·m L-1、1.945ug·m L-1and 383.5×10~9·L-1。Conclusions Serum of RDW,PCT,PLT and D-D were the risk factors of KD combining with CALs.The combined prediction of serum of RDW,PCT,PLT and D-D was of higher clinical value in KD combining with CALs. |
