Efficacy Analysis Of Lateral Single Plate Or Additional Anterior Medial Plate Fixation For Comminuted Distal Femoral Fracture

ObjectiveThe treatment of distal femur comminuted fracture is difficult.The purpose of this study was to analyze the methods and efficacy of using lateral single plate or additional anterior medial plate fixation in the treatment of distal femur fracture in our hospital,so as to provide reference for clinicians.MethodsA total of 51 cases of distal femur comminuted fracture treated with lateral locking plate or additional anterior medial plate fixation in our hospital from june2018 to january 2021 were retrospectively analyzed,including 30 males and 21 females,aged from 22 to 82 years old,with the average age of（55.88 ±14.12）years old,among which 21 cases were A3 type,18 cases were C2 type,12 cases were C3 type,36 people were injured by falling,8 people were involved in a car accident,and7 people were injured by falling from a height,smashing and other factors.According to whether there is support on the medial side of the distal femoral comminuted fracture,single plate or double plate fixation was selected to analyze the treatment effect.ResultsThe follow-up ranged from 9 to 36 months,with an average of 18.7 months.all the wounds healed in one stage.there were two cases complicated with deep vein thrombosis of lower limbs,and one case complicated with pulmonary infection and one case complicated with urinary tract infection.During the follow-up,there were 1case of plate fracture,1 case of bone nonunion and 2 cases of delayed union,and the healing rate was 96.1%.Postoperative Kolmert efficacy evaluation showed excellent in 15 cases,good in 31 cases,fair in 3 cases,and poor in 2 cases,with the total excellent and good rate of 90.2%.ConclusionSingle lateral plate or additional anterior-medial plate fixation for the treatment of comminuted distal femoral fracture has good clinical efficacy.The specific fixation method should be selected according to the characteristics of the fracture.Double plate fixation is recommended when the medial side lacks structural support,especially when there is bone defect.
