Objective To investigate the influencing factors of ventricular remodeling after percutaneous coronary intervention(PCI)in patients with acute ST-segment elevation myocardial infarction.Methods Retrospectively,100 patients with acute ST-segment elevation myocardial infarction admitted to our hospital from June 2020 to June 2021 and underwent PCI treatment within 12 hours were retrospectively selected as research subjects,and based on the results of cardiac ultrasound after 6 months of admission and follow-up,the ventricular remodeling group(n=39)and the non-ventricular remodeling group(n=61)were analyzed.According to the comparison of clinical data of each group and UA,CysC and other related indicators,the influencing factors of ventricular remodeling were analyzed.Logistic regression was used to analyze the risk factors associated with ventricular remodeling in patients with PCI in myocardial infarction.The predictive value of UA and CysC for ventricular remodeling in patients after PCI of myocardial infarction was analyzed by ROC curves.Results(1)Comparison of basic data between ventricular remodeling group and non-ventricular remodeling groupThe number of diabetic patients,CysC and uric acid serum levels,the number of coronary artery lesions,and the number of patients with anterior wall infarction in the ventricular remodeling group were significantly higher than those in the non-ventricular remodeling group,and the differences were statistically significant(P<0.05)..There were no significant differences in age,gender,smoking history,hypertension history,HDL-C,LDL-C,TG,TC,CK-MB,hs-cTnI,NT-proBNP serum levels(all P>0.05)(2)Comparison of serum levels of uric acid and cystatin C among different coronary artery disease branch groupsThe serum uric acid and CysC levels in the multi-vessel disease group were significantly higher than those in the single-vessel disease group and the double-vessel disease group,and the difference was statistically significant(P<0.05).(3)Logistic regression analysis took ventricular remodeling as the dependent variable,and included significant relevant indicators into the multivariate logistic regression model to screen independent predictors.The results showed that uric acid(OR=1.126,95%CI 1.083,1.170),cystatin C(OR=15.133,95%CI 5.116,44.758),infarct site(OR=6.354,95%CI 1.446,27.924)were Independent risk factors of ventricular remodeling after PCI in myocardial infarction(P<0.05).(4)The results of receiver operating characteristic(ROC)curve analysis showed that the area of the ROC curve for CysC to predict ventricular remodeling was 0.831(95%CI:0.751,0.910,P<0.001),and the ROC curve for uric acid to predict ventricular remodeling in patients was The lower area was 0.781(95%CI:0.686,0.876,P<0.001).Conclusion1.Uric acid and cystatin C may be risk factors for ventricular remodeling after PCI in acute myocardial infarction,and may be used as indicators to predict the occurrence of ventricular remodeling after PCI in acute myocardial infarction.2.The predictive value of cystatin C on ventricular remodeling after PCI in acute myocardial infarction may be better than that of uric acid.
