| Research Purpose:The purpose of this study was to construct an intervention program for unmet needs in patients undergoing first chemotherapy for advanced lung cancer based on ERG theory and to investigate the effects of the program on unmet needs,positive and negative emotions,and quality of life in patients undergoing first chemotherapy for advanced lung cancer.The aim is to provide a practical reference and theoretical basis for the future management of patients with advanced lung cancer treated with chemotherapy for the first time.Research Methods:1 Under the guidance of ERG theory,the first draft of an intervention protocol for unmet needs of patients with advanced lung cancer on first chemotherapy based on ERG theory was constructed through literature review,semi-structured interviews and study group discussions,and then the intervention protocol was determined through two rounds of Delphi expert consultation.2 We used a non-concurrent controlled trial approach in a class of experimental studies to implement a modified unmet need intervention protocol for patients with advanced lung cancer treated with first-time chemotherapy.The thirty patients included from April to July 2021 were used as the control group and 29 patients included from August to November 2021 were used as the intervention group,with the control group given the usual care regimen and the intervention group implementing this intervention regimen for one chemotherapy cycle.Baseline data were collected,and post-intervention patient scores on the SF-SUNS,PANS and FACT-L.The collected data were statistically described and statistically inferred through SPSS 24.0 software.Research Results:1 An intervention program for unmet needs of patients with advanced lung cancer undergoing their first chemotherapy was constructed,in which the pre-chemotherapy phase includes interventions for unmet disease information needs,unmet needs for quality of medical care visits and unmet needs for emotional support from healthcare professionals in 3 areas,the chemotherapy phase included interventions for unmet information needs for efficacy and safety of chemotherapy regimens,unmet information needs for self-management of intravenous access to chemotherapy,unmet needs for financial support,and unmet needs for peer support education in four areas.The post-chemotherapy phase is an intervention for patients with unmet needs for management support in the hospital-family-community triad,with 31 detailed interventions.2 We validated the effect of an unmet need intervention regimen for first chemotherapy in patients with advanced lung cancer2.1 Comparison of baseline data:Before intervention,the general information,degree of unmet needs,positive and negative emotional status and quality of life of patients in the two groups were comparable(P> 0.05).2.2 Intergroup comparison of each data between the two groups after the intervention:After the intervention,the SF-SUNS(t=6.652,P<0.001),PANS(t=4.952,P<0.001 and t=3.916,P<0.001)and FACT-L(t=13.262,P<0.001)values were better in the intervention group than in the control group,and all differences were statistically significant,however,there were no statistical differences between groups in the data for both the information unmet need dimension and the economic unmet need dimension of the post-intervention SF-SUNS(P>0.05).2.3 Comparison of the each data in the intervention group pre and post-intervention:After the intervention,the SF-SUNS(t=15.484,P<0.001),PANS(t=11.661,P<0.001 and t=13.585,P<0.001)and FACT-L(t=28.466,P<0.001)were better in the intervention group than before the intervention,and the differences were statistically significant.However,there was no statistically significant difference in the additional status dimensions of the FACT-L in the intervention group before and after the intervention(P>0.05).Research Conclusion:Based on the ERG theory,this study constructed an unmet need intervention plan for the first chemotherapy of patients with advanced lung cancer.After clinical verification,it can not only reduce the unmet needs and negative emotions of patients with advanced lung cancer during the first chemotherapy,but also improve positive emotions and improve Quality of life is a scientific,feasible and reliable intervention program,which can provide reference for the management of the first chemotherapy in patients with advanced lung cancer,and is worthy of clinical application. |
