Clinical Observation On Yishen Xiezhuo Decoction In The Treatment Of Qi And Yin Deficiency Syndrome Of Chronic Kidney Disease Stage 3-4

Objective: In this study,we observed the clinical efficacy of Yishen Xiezhuo Decoction in the treatment of chronic kidney disease with Qi and Yin deficiency syndromes of stage 3-4,and evaluated its efficacy and safety,so that the prescription can be fully and flexibly used in the treatment of chronic kidney disease.Method: For the 60 patients who meet the inclusion criteria,according to the principle of randomization and control,using the RAND function in Excel 2019,they were divided into a control group and a test group with 30 cases each.The control group received basic treatment,and the experimental group received basic treatment plus Yishen Xiezhuo Decoction for 12 weeks.Serum creatinine(Scr),estimated glomerular filtration rate(e GFR),cystatin C(Cys-C),blood urea nitrogen(BUN),uric acid(UA),24 hours urine total protein(24h UTP)and other related safety indexes were recorded before and after treatment in the two groups,so as to determine the clinical efficacy and TCM syndrome efficacy.All data were statistically analyzed by SPSS23.0.Results: 1.In terms of clinical efficacy,the total effective rates of the test group and the control group were 83.33% and 66.67%,respectively,and the test group had more effective efficacy than the control group(P<0.05).2.In terms of the efficacy of TCM syndromes,the total effective rate of the test group was 86.67%,and that of the control group was 56.67%.The scores of TCM syndromes of the test group and the control group decreased(P<0.01),and the test group decreased more significantly than the control group(P<0.05).Except for vertigo and tinnitus,the test group can reduce the scores of the other 7 TCM syndromes(P<0.05),and the control group can only reduce the TCM syndrome scores of dry mouth and throat(P<0.05).3.In terms of laboratory efficacy indicators,the test group significantly reduced the 24 h UTP level(P<0.01),Scr,BUN,Cys-C,UA,hemoglobin(Hb),and urinary red blood cells(URBC)were not significantly improved in the two groups(P>0.05),but in the change values of Scr,e GFR,Cys-C,and the rate of change of Scr and e GFR,the test group has more obvious changes than the control group(P<0.05).There was no significant difference in the changes between the two groups(P>0.05).4.In terms of safety indexes,potassium ion(K),prothrombin time(PT),activated partial thromboplastin time(APTT),aspartate aminotransferase(AST)and alanine aminotransferase(ALT)groups before and after treatment were not statistically significant(P>0.05).Conclusion: 1.Yishen Xiehuo Decoction can significantly reduce the level of urinary protein in patients with Qi and Yin deficiency syndrome at stage 3-4 of chronic kidney disease.Compared with basic treatment,Yishen Xiehuo Decoction can delay the increase of serum creatinine and cystatin C and the decrease of glomerular filtration rate.2.Yishen Xiezhuo Decoction can significantly improve the clinical symptoms of patients with chronic kidney disease 3-4 stage Qi and Yin deficiency syndrome,mainly manifested in fatigue,fatigued spirit and lack of strength,lack of qi and no desire to speak,spontaneous sweating,poor appetite,night sweats,dry mouthv and throat,feverish sensation over the five centers.3.Yishen Xiezhuo Decoction has better safety in the treatment of 3-4 stage Qi and Yin deficiency syndrome of chronic kidney disease.
