ObjectiveTo study and analyze the TCM syndrome types and clinical characteristics,and the correlation between TCM syndrome and the risk of cardiovascular events in maintenance hemodialysis patients with CKD,so as to investigate the risk of cardiovascular events in different TCM syndromes and provide theorical evidence for the individual TCM prevention and treatment on those patients with high risk of cardiovascular events.Methods120 patients with maintenance hemodialysis CKD were included in this study.By collecting the general situation of the patients and the four diagnostic information of traditional Chinese medicine,the TCM syndrome distribution characteristics of MHD patients were analyzed,and the differences of clinical characteristics of different TCM syndrome types were discussed.The results of laboratory examination(LDL-C,HDL-C,BNP,Hcy,etc.)within 6 months were inquired to analyze the correlation between TCM syndrome types and cardiovascular disease risk,and to provide clinical reference for TCM dialectical prevention and treatment of cardiovascular events.ResultsAmong 120 cases of maintenance hemodialysis CKD patients,36 cases(30%)were diagnosed as liver and kidney yin deficiency syndrome,followed by 23 cases(19.17%)of spleen and kidney Qi deficiency syndrome,18 cases(15%)of Qi and yin deficiency syndrome,17 cases(14.17%)of spleen and kidney yang deficiency syndrome,16 cases(13.33%)of damp-heat syndrome,and 8.33% of blood stasis syndrome.There were significant differences in age,BMI,cholesterol and LDL-C among different syndrome types of maintenance hemodialysis CKD patients(P<0.01);the average age of patients with spleen and kidney Qi deficiency syndrome,spleen and kidney yang deficiency syndrome and Qi and yin deficiency syndrome were higher than those of CKD patients with damp-heat syndrome and blood stasis syndrome(P<0.05),and higher than those of CKD patients with liver and kidney yin deficiency syndrome(P<0.05);the overall BMI of patients with damp-heat syndrome was higher than that of CKD patients with liver and kidney yin deficiency syndrome(P<0.05).The BMI of patients with spleen kidney yang deficiency syndrome was higher than that of patients with other syndromes(P<0.05);the cholesterol and LDL-C of patients with damp-heat syndrome were higher than those of other syndromes(P<0.05);the BNP level of CKD patients with various syndromes increased with time,especially in patients with spleen kidney yang deficiency syndrome,which was higher than that of other syndromes(P<0.01),and the growth trend was more obvious.There was a strong positive linear correlation(r=0.9112).There was a moderate positive linear correlation between hemodialysis time and AIP level in CKD patients with blood stasis syndrome(r=0.6031),and a strong positive linear correlation between hemodialysis time and AIP level in CKD patients with spleen kidney yang deficiency syndrome(r=0.7594).The baseline level of Hcy in CKD patients with spleen and kidney Qi deficiency,spleen and kidney yang deficiency,damp-heat syndrome and blood stasis syndrome was higher,and the trend was obvious with the extension of dialysis time.ConclusionCKD patients with different syndrome types have different characteristics in general conditions,eating habits,basic diseases,laboratory examination,etc.,and the risk of cardiovascular events is very different.Patients with spleen and kidney yang deficiency syndrome,spleen and kidney qi deficiency syndrome,and blood stasis syndrome have higher risk of cardiovascular events.Personalized Chinese medicine intervention can prevent and treat the cardiovascular events of these patients It may have positive clinical significance. |
