| Purpose :To investigate the clinical effect of biological mesh combined with transvaqinal hyattreetony in the treatment of senile uterine prolapse,discuss the application value of 3D-MRI in pelvic prolapse.Methods: From December 2016 to December 2017,60 patients of uterine prolapse with POP-Q grade II or above in the Gynecology of Jiangyin People’s Hospital were recruited to the research.They were randomly divided into the control group and the experimental group according to the random number table,30 cases each.The control group was treated with transvaqinal hyattreetony plus vaginal anterior and posterior wall repair,and the experimental group was treated with biological mesh on the basis of the control group.The surgical condition including operation time,intraoperative bleeding volume,indwelling time of catheter,residual urine volume and hospitalization time was observed.The scores of Aa,Ba,C,Ap and Bp of POP-Q indexes were recorded before treatment and at 12 months after treatment.Also the changes of quality of life(PFDI-20,POPDI-6,CRADI-8 and UDI-6)and the results of3D-MRI were compared in the two groups.Results: 1.There were no significant difference in operation time,intraoperative bleeding volume,indwelling time of catheter,residual urine volume and hospitalization time in the two groups(P>0.05).2.Scores of POP-Q、PFDI-20、POPDI-6、CRADI-8 and UDI-6 were better after treatment both in the control group and the experimental group(P<0.05).3.In the experimental group,POP-Q scores of Aa,Ba,C,Ap and Bp were better,the scores of PFDI-20,POPDI-6,CRADI-8 and UDI-6were lower,the M line of resting position and Valsalva action position was smaller,and the C line was larger than those in the control group after treatment(P<0.05)after treatment.4.There were no significant difference in PCL,H line,G1 angle and G2 angle(P>0.05),while the B line of Valsalva action position was larger than that in the control group(P<0.05).Conclusions: 1.Surgical treatment can significantly improve the quality of life of patients with uterine prolapse.2.Biological mesh could improve POP-Q scores,and improve pelvic floor structural defects and quality of life after operation in senile patients with uterine prolapse compared with traditional surgical approach.3.3D-MRI can be used for preoperative and postoperative evaluation of patients with pelvic prolapse. |
