| Objective: To observe the effect of Tongtian oral liquid and its effect on CGRP,SP,5-HT in the treatment of chronic migraine with blood stasis and phlegm by comparing the effect of Tongtian oral liquid and Rizatriptan benbenate tablet in the treatment of acute attack period and the change of serum CGRP,SP,5-HT before and after treatment.Therefore,the mechanism of Tongtian oral liquid in treating chronic migraine is discussed,and the experimental direction is provided for the further study of the mechanism of action of traditional Chinese medicine.METHODS:(1)Eighteen patients with chronic migraine who met the inclusion criteria were selected,and the methods of randomized,double-blind,double-simulation,positive drug control,self before and after cross-control were used.Finally,16 cases were tested.(2)Drug administration: All patients were given(1)Tongtian Oral Solution +Riza Triptan Benzoate Tablets simulation agent or(2)Riza Triptan Benzoate Tablets +Tongtian Oral Solution simulation agent in treatment period 1 and 2,respectively,and the order of medication was random.A washout period of 14 days was set during treatment.(3)Efficacy indicators: Visual analogue scale(VAS)was used to evaluate the headache degree before and 1h,2h and 4h after medication,respectively.The VAS was effectively reduced by more than 50% or moderate to severe to mild.In the statistical analysis,they were divided into the traditional Chinese medicine group(Tongtian oral liquid group)and the western medicine group(Liza Triptam group)for inter-group and intra-group comparison.(4)Blood indicators: CGRP,SP and 5-HT were used in cubital vein blood indicators during the interval of attack,before and 4h after medication.(5)Safety indicators: ECG,liver and kidney function,and blood routine tests were performed before and after medication.(6)Collected data were sorted out and analyzed with SPSS26.0.Results :(1)Compared with before treatment,VAS scores of the two groups at 1h,2h and4 h after treatment decreased gradually(P<0.01);The improvement degree of 1h score in the western medicine group was better than that in the Chinese medicine group(P<0.05),and there was no significant difference in the improvement degree of 2h and4 h score between the two groups(P>0.05).(2)Effective rate comparison: 1h and 2h effective rate in TCM group was lower than that in western medicine group,but the difference was not statistically significant(P > 0.05).4h effective rate: the Chinese medicine group and the western medicine group were the same,the effective rate of both groups was 68%.(3)Compared with the intermittent period,serum CGRP,SP and 5-HT contents had no significant difference(P>0.05),but the contents of CGRP and SP showed an upward trend,while the contents of 5-HT showed a downward trend.(4)Serum CGRP and SP contents were decreased in both groups before and 4h after medication(P<0.05).Serum 5-HT content was increased(P<0.05).There were no significant differences in the contents of CGRP,SP and 5-HT between the two groups(P>0.05).Conclusion :(1)Tongtian oral liquid is effective in the treatment of chronic migraine with blood stasis and wind,and there is no significant difference in the treatment effect compared with Lizatriptam.(2)Tongtian oral liquid can reduce serum CGRP and SP content and increase 5-HT content in the onset of chronic migraine with blood stasis and chiphon,which may be the mechanism of Tongtian oral liquid in the treatment of headache in the onset of chronic migraine with blood stasis and chiphon.(3)There was no significant difference in serum CGRP,SP and 5-HT levels between the acute and intermittent periods of chronic migraine,but the levels may change with the relief of headache after treatment. |
