Literature Analysis And Data Mining Of Cough Cases In Medical Records Integration Of Palace In Qing Dynasty

ObjectiveThe purpose of this research is to sort out,analyze,and research the cough records in the Qing palace medical records.Discuss the treatment law of cough in Qing palace medical records.On the one hand,it promotes the thematic research of Qing palace medical records,and on the other hand,it provides more references for modern Chinese medicine clinical cough diagnosis and treatment.MethodsIn this study,based on Medical Records Integration of Palace in Qing Dynasty edited by Chen Keji and others published by Beijing Science Press in 2009,85 cough cases were screened out,and the literature analysis and data mining techniques were used for research.First of all,using the literature analysis method to extract several major pathogenesis of cough in the medical records of the Qing Dynasty.Then,through specific case analysis,it explains the development and changes of its syndrome types and the process of syndrome differentiation and treatment.Next,we use data mining technology to study cough prescriptions,and extract 366 prescriptions in cough medical records according to certain inclusion and exclusion criteria.We standardize and unify drug names.In addition,the TCM inheritance assistance platform(V3.0)was used to conduct data mining for "drug efficacy category","medication frequency","four-gas frequency","five-flavor frequency","channel frequency","common drug pairs and drug groups","cluster analysis","drug core network" and other aspects.Through the analysis of relevant studies and data mining,the basic contents of cough pathogenesis and prescriptions in medical records of Qing Dynasty were deeply studied,and the diagnosis and treatment characteristics of cough cases in medical records of Qing Dynasty were summarized by induction and summary method.Conclusion1.The pathogenesis of cough in Qing Gong medical records mainly includes 4categories: cough with exogenous infection,cough with phlegm,liver and lung discord,and weakness of the spleen and stomach.Among them,exogenous coughs are the most common,and the changes in syndrome types are the most complicated.Exogenous cough includes 5 categories: wind chill,wind temperature,dryness,summer dampness and chronic diseases induced by exogenous cough.Among them,the evolution of windcold cough is the most complex and diverse,covering five kinds of changes: heavy wind chill,equal phlegm and drink,outside wind chill,heavy phlegm and drink,outside wind chill,stagnation and heat in lung and stomach,stagnation and depression in wind chill,phlegm-heat interaction.At the beginning of the wind and cold,the main method is to relieve the surface,release the lungs and relieve cough.With the appearance of pathological changes such as internal movement of phlegm and drink,stagnation of heat in the lungs and stomach,and mutual accumulation of phlegm and heat,different treatment methods such as xuanhua phlegm,clearing heat and eliminating stagnation or guiding stagnation,clearing heat and resolving phlegm,etc.are combined.And adjust the proportion according to the severity of the disease.Wind-warm cough is internally caused by insufficient Shaoyin fines,so there is a difference between exogenous wind temperature and insufficient Yin fines.In the treatment method,it is distinguished mainly to clear away heat and remove evil,and to clear away heat and nourish yin.Later pay attention to clearing heat and resolving phlegm.There are external dryness and internal dryness in Zao Xie cough.External dryness should be cleared and moisturized,and internal dryness should be moisturized to dissolve dryness.Shushi cough is distinguished by the heavier summer evil and the heavier wet evil.The former is suitable for replenishing qi to relieve heat,while the latter is suitable for regulating qi and dissipating dampness.In the cough caused by exogenously induced chronic diseases,while dispelling the evil and relieving the surface,the chronic diseases are regulated.Phlegm cough is the second major type of cough in Qinggong medical records,including phlegm-damp cough and phlegm-heat cough.The former is based on the principle of promoting qi,removing dampness and transforming dampness,while the latter is based on the principle of clearing heat and resolving phlegm,and promoting yang qi.Liver-pulmonary discordant cough mainly includes three types of liver failure,qi drink upset,stagnation of heat in the liver channel,wood fire penalty,insufficient liver-yin,and liver-yang flushing the lungs.Respectively treat the Yin by regulating Qi and Jiangni Huayin,soothing the liver,calming the liver and clearing away heat,and nourishing the liver and lungs.Cough caused by weakness of the spleen and stomach is mainly manifested in two aspects: the soil does not produce gold and the spleen dampness invades the lungs.The main treatment is to invigorate the spleen and dispel dampness.In addition,cough medical records in Qing Palace also include several special types of cough,tuberculosis,pregnancy cough,and pediatric cough.2.Set the quantity of support as 37 and confidential level as 0.5,use the assistant platform for the heritage of Traditional Chinese Medicine(V3.0)to acquire the result of data mining.The 366 prescriptions for the medical records of Qing Palace related to cough involve the efficacy of 16 kinds of drugs of which the use frequency of drugs for resolving Phlegm and relieving cough and asthma is the highest with the drugs in the class of vacuity supplement and heat clearing taking the second place.The drug is the most cold,warm second,most of the medicinal taste is bitter,followed by spicy,with the dominance of lung meridian for channel tropism and spleen meridian taking the second place.There are 13 drugs of which the use frequency is over 100.Specifically:213 times for licorice,169 times for platycodon grandiflorum,163 times for almond,145 times for pinellia ternate,142 times for scutellaria baicalensis,139 times for citron fruit,128 times for tangerine,122 times for dried tangerine or orange peel,121 times for the root of purple-flowered peucedanum,112 times for fritillaria thun-bergli,106 times for Ophiopogon japonicus,105 times for the root bark of white mulberry,100 times for poria cocos.The core combination of drugs obtained when the couplet medicines,drug group and cluster number are 4 is mainly composed of the above drugs of high frequency.The couplet medicines with the use frequency of over 80 are:platycodon grandiflorum with almond for 100 times and scutellaria baicalensis with citron fruit for 89 times.The platycodon grandiflorum with almond demonstrates the application of method with light bitterness and pungency.The scutellaria baicalensis with citron fruit demonstrates the application of combination with drugs of bitterness and coolness and drugs of pungency and mansuetude.3.Combining the results of literature analysis and data mining,the characteristics of cough diagnosis and treatment in Qing Palace medical records are summarized as follows: First,pay attention to pulse diagnosis and search for the primary cause of disease in treatment.Second,flexible use of famous prescriptions at the time.Third,use cold and warm together,bitter and spicy together.Fourth,based on the lungs and focus on the balance of the five internal organs.Fifth,regulate body fluid and qi with invigorating the circulation of blood.Sixth,pay attention to physique and protect yin points.Seventh,flexibly apply guiding drug.
