| BackgroundLumbar,leg pain is lumbar disc herniation,the most common symptoms due to internal degenerative changes in the lunbar spine and external factors such astrauma,strain,cold and wet.Non-surgical or surgical treatment is performed according to the severity of clinical symptoms.percutaneous foraminal surgery is a commonly used surgical method at present.The advantages of this surgica method are less trauma,less bleeding low cost and short hospitalization period.On the other hand,the incidence of perioperative adverse events and complications has increased.Therefore,developing and studying fine nursing has a positive impact on perioperative risk prevention and reduction of complications in patients with lumbar disc heniation.ObjectiveTo study the effect of fine nursing on deep vein thrombosis of lower extremity in patients with disc herniatition on treated by intervertebral foraminoscopy,To explore the effect of fine nursing on perioperative indexes of patients with Lumbar disc herniation and poetoperative complications.MethodsFrom February 2019 to April 2020,120 patients with intervertebral disc herniation who underwent percutaneous foraminal endoscopic surgery were selecteed in this study.The patients were randomly divided into two groups(n=60)as the fine nursing group(observation group)and conventional nursing group(control group).according to the envelope method.Based on the nursing of the control group,the patients in the observation group were given specific analysis for the risk links of complications and refined overall nursing.Before and after nursing,the indexes of patients in fine nursing group and conventional nursing group were observed:the ratio of CD4~+T lymphocytes and CD4~+CD25~+Tlymphocytes in peripheral blood was detected by flow cytometry,the level of serum high-sensitivity C-reactive protein(hs-CRP)was detected by immunoturbidimetry,and the level of serum interleukin-6(IL-6)was detected by ELISA.The pain degree from 24 hours before operation to 48 hours after operation was evaluated by visual analog digital method,the perioperative lumbar function was evaluated by Lehmann score,the perioperative quality of life was evaluated by quality of life scale SF-36,and the incidence of postoperative malignant adverse events in the two groups was compared and analyzed with the help of auxiliary examinations of medical technology departments such as ultrasound and imaging.ResultsThe variation range of heart rate and mean arterial pressure in fine nursing group was better than that in conventional nursing group at different time points,(P<0.05).After inter foraminal endoscopic surgery,the peroperative pain score of patients in the observation group was lower than that in the control group and the SF-36 score was higher than in the control group,(P<0.05).After inter foraminal endoscopic surgery,the levels of serum IL-6 and hs-CRP in the observation group were decreased,and the postoperative decrease was better than that in the contral group,(P<0.05).After inter foraminal endoscopic surgery,the number of CD4~+T lymphocytes in the peripheral blood of patients in the observation group increased,while the ratio of CD4~+CD25~+T lymphocytes in perpheral blood of patients in the observation group decreased.Compareed with before treament,the observation group was better than the contral group,(P<0.05).After foraminal endoscopic surgery,the iprovement of lumbar function in the midterm observation group was better than that in the control group,(P<0.05).Comparis on of the incidence of posterative malignant adverse events between the two groups:after the treatment of lumbar disc herniation by percutaneous foraminal surgery through fine nursing,it was found that the incidence of complications such as delusions infection,cerebrovascular accident and acute coronary syndrome was significantly reduced,and the length and invidence of lower extremity deep vein thrombosis were significantly decreased.the difference was statistically significant compared with the control group,(P<0.05).ConclusionFine nursing can reduce the formation of deep venous thrombosis in heart limbs,and its mechanism may be related to reducing inflammatory cells and reducing the expression of disease-like factors.For percutaneous intervertebral formen mirror treatment of lower extremity deep wein thrombosis in patients with postperative,improve the patients and families to peroperative care of indicators of satisfaction,reduce the pain index,improve the postoperative affect influence,isbeneficial the physical and mental health of patients. |
