ObjectiveThis study aims to investigate the level of fear of cancer recurrence,and to explore its relationship with positive and negative emotions,psychological capital,and social support in patients with acute leukemia,so as to provide theoretical basis for reducing the fear of cancer recurrence and provide guidance for implementing the effective psychological intervention.MethodsA total of two hundred and twenty-nine patients with acute leukemia from department of Hematology of three tertiary A hospitals in Shandong Province were selected by convenient sampling method.The survey tools included general information questionnaire,FOP-Q-SF,PANAS,PSSS and PPQ.Using the statistical software of SPSS 22.0,we carried out the descriptive statistical analysis,independent sample t-test,one-way ANOVA and Pearson correlation analysis.we constructed the structural equation model between the fear of cancer recurrence and related factors by Amos 22.0.Results1.Demographic and clinical data in patients with acute leukemiaThe two hundred and twenty-nine patients with acute leukemia were ranged from eighteen to seventy-seven years old.The research found that an average age of them was(41.68 ± 12.68)years old,and the proportion of male(49.3%)and female(50.7%)was similar.Most of them were married(82.1%)and employed(55.5%),and had a junior high school degree or below(48.0%).The proportion of those whose income was not enough to make ends meet(51.5%)was the largest and those whose income was enough to make ends meet(11.4%)was the smallest.Most patients were coming from rural areas(55.5%),living with their families(87.8%),and had children(85.6%).The majority of them had no chronic history(66.8%),and the main caregivers of them were spouses(53.7%).Most of the patients were acute myeloid leukemia(79.0%).The course of disease was 3 months(42.4%).Most of the patients were in remission after treatment(56.3%).The proportion of patients with a family history(5.7%)was very small.2.Analysis of differences in demographic and clinical data of fear score of cancer recurrence in patients with acute leukemiaIn this study,the score of fear of cancer recurrence in patients with acute leukemia was(41.60±8.30).Thereinto,the score of physical health dimension was(21.34 ±4.32)and the score of social family dimension was(20.27±4.55).One hundred and ninety-four patients’ scores of fear of cancer recurrence were no less than thirty-four,which accounted for 84.7%.Univariate analysis showed that there were statistically significant differences in the total score of fear of cancer recurrence and the scores of each dimension in the patients with acute leukemia in marital status,economic status,treatment stage,having children or not and having family history or not(all P<0.05).3.Correlation analysis of fear of cancer recurrence with psychological capital,positive and negative emotions,and social support in patients with acute leukemia.Pearson correlation analysis showed that total scores of fear of cancer recurrence and its two dimension scores were negatively correlated with the score of positive emotion,total scores of psychological capital and its each dimension scores,total scores of social support and its each dimension scores(r=-0.176~-0.433,P<0.01).Besides,it was positively correlated with the score of negative emotion(r=0.392~0.427,P<0.01).4.Analysis of structural equation model on related factors of fear of cancer recurrence in patients with acute leukemia.Statistical software of Amos22.0 is used to construct the structural equation model of relevant factors.According to above results,the model fitted well.The results displayed that social support could directly affect the fear of cancer recurrence(direct effect coefficient was-0.296)and also could indirectly affect the fear of cancer recurrence through positive emotion(indirect effect coefficient was-0.064),but psychological capital could directly affect the fear of cancer recurrence through positive and negative emotions(indirect effect coefficient was-0.157).Conclusion:1.The proportion of patients with acute leukemia whose level of fear of cancer recurrence reached the clinical standard was high,which was worthy of clinical attention.2.The score of fear of cancer recurrence in patients with acute leukemia inordinately had negative correlation with psychological capital,social support,and positive emotion,and had positive correlation with the score of negative emotion.3.Psychological capital in patients with acute leukemia could indirectly affect the fear of cancer recurrence through positive and negative emotions.Social support could directly affect the fear of cancer recurrence,also could indirectly affect it through positive emotion.
