Objective:To analyze the clinical features of large artery atherosclerosis stroke(LAA-S)on the original inhabitants of Han in Hainan area,to discuss the risk factors,imaging characteristics and distribution of vascular lesions,to evaluate the prognosis of LAA-S patients,so as to enhance our level of diagnosis and prevention of LAA-S patients The original inhabitants of Han in Hainan area are relatively isolated。As far as we know,up to now,there is no large-scale clinical research about LAA-S on them.Methods:According to TOAST etiology classification method,We selected305 LAA-S on patients admitted to The Second Affiliated Hospital of Hainan General Hospital,Haikou People’s Hospital,the third people’s Hospital of Haikou,Chinese People’s Liberation Army 187 hospital,the First Affiliated Hospital of Hainan Medical College,and the Second Affiliated Hospital of Hainan Medical College from August 2015 to December 2016.All patients were divided into two groups,under the age of 60 and over the age of 60,the personal history,the family history of cerebrovascular disease,and the results of laboratory tests and imaging examinations during hospitalization were collected.Statistical analysis was performed by using SPSS 22.0.T test and the mono-factor chi-squared test were used to analyze the risk factors,imaging findings,distribution characteristics and prognosis of vascular lesions in patients with LAA-S.Results:There were 227 males and 78 females,average ages is59.5 years old(SD11.9)with 195 cases of patients over 60 years old and 110cases of patients under the age of 60.It was not statistically significant(χ~2=0.732,P=0.392)between the two groups of patients with gender or age difference,but male patients were more common than female patients.All patients with hypertension(70.5%,215/305)is the most common,followed by dyslipidemia(62.2%,190/305),followed by diabetes(52.1%,159/305),family history of cerebrovascular disease(39.6%,121/305),smoking(36.7%,112/305),high fibrinogen levels(35.4%.108/305),alcohol(27.8%,85/305).The most frequently located in the basal ganglia in patients with cerebral infarction lesions accounted for all patients(54.1%),Multiple cerebral infarction is more common in 60~89 years group(χ~2=8.409,P=0.004)).This two groups had significant differences and 30~60 years group is better than 60~90 years group in prognosis.The most frequently located in the basal ganglia in patients with cerebral infarction lesions accounted for all patients(54.1%).The two age groups,no statistically significant differences in infarct area in patients with LAA-S,but found more common multiple infarction patients over the age of 60(χ~2=8.409,P=0.004).220 cases of patients with intracranial large artery disease,significantly more than extracranial arterial lesions in 85 cases;276 cases of patients with anterior circulation lesions,more than posterior circulation vascular lesions in 29 cases.Conclusion:LAA-S patients are more common in male patients,and the incidence is significantly higher after 60 years of old.All patients with hypertension is the most common,followed by dyslipidemia,followed by diabetes,family history of cerebrovascular disease.Blood lipid abnormality are more common in 60~89 years group.The most frequently located in the basal ganglia in patients with cerebral infarction lesions,Multiple cerebral infarction is more common in 60~89 years group.LAA-S patients with intracranial vascular lesions are more than extracranial vascular and anterior circulation vascular lesions are significantly more than the posterior circulation of blood vessels.30~60years group has a better prognosis. |
