| Objectives By comparing the changes of serum Ghrelin and Obestatin expression levels in patients with functional dyspepsia(FD)before and after mirtazapine treatment,the correlation between Ghrelin and Obestatin expression levels and symptom degree,life quality,psychological factor in patients with FD was analyzed,to explore the possible role of mirtazapine in the treatment of FD.Methods Thirty cases of FD patients with decreased appetite and weight loss who were treated in the Gastroenterology Department of Hebei General Hospital from June 2020 to December 2020 and met the Rome IV diagnostic criteria were included as the experimental group(FD group for short),and Thirty healthy people who underwent routine physical examination at Hebei General Hospital during the same period were included.Example as a control group(N group for short).Relevant clinical data were recorded and serum samples were collected.The expression levels of Ghrelin and Obestatin in fasting serum of all subjects were detected by ELISA,and the Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS)and Nepean Dyspepsia Index(NDI)scores were evaluated.Follow-up was performed 4 weeks after mirtazapine treatment in FD group(FD group after treatment for short),and fasting serum Ghrelin and Obestatin expression levels,SAS,SDS,NDI scores in FD group after treatment were detected.The independentsamples T test was used to compare the difference of serum Ghrelin Obestatin expression level,SAS,SDS,NDI scores between FD group before treatment and N group,the pairedsamples T test was uesd to compare serum Ghrelin Obestatin expression level,SAS,SDS,NDI scores between the FD group before and after treatment,Pearson correlation analysis was uesd to evaluate the correlation between serum Ghrelin Obestatin expression level and NDI scores in FD group before and after treatment.Results 1 Analysis of basic data of subjects: the differences in age and sex between FD group and N group were consistent,there was no statistical significance(P>0.05).The differences in BMI was statistically significant(P<0.05).2 Comparison of serum Ghrelin and Obestatin expression levels in FD group before treatment and N group: the serum Ghrelin expression levels in FD group before treatment and N group were 4.27±1.79ng/ml and 6.26±2.19ng/ml,respectively,and the serum Ghrelin expression levels in FD group before treatment were significantly lower than that in N group(P<0.05).Serum Obestatin expression levels in FD group before treatment and N group were 36.70±9.37ng/ml and23.23±7.49ng/ml,respectively.Serum Obestatin expression levels in FD group before treatment were significantly higher than those in N group(P<0.05).3 Comparison of SAS,SDS,NDSI and NDLQI scores between FD group before treatment and N group: The SAS,SDS,NDSI and NDLQI scores of FD group before treatment were 53.17±11.12,51.43±11.62,51.23±12.66,54.13±15.63,respectively;the SAS,SDS,NDSI and NDLQI scores of N group were 40.43±10.39,35.77±4.42,5.87±3.32,95.27±3.06,respectively;the SAS,SDS and NDLQI scores of FD group before treatment were significantly higher than those of N group(P<0.05).NDLQI score was significantly lower than that in N group,and the difference was statistically significant(P<0.05).4 Correlation analysis between serum Ghrelin expression level and NDSI,NDLQI scores before treatment: serum Ghrelin expression level was negatively correlated with NDSI score(r=-0.727,P<0.05),and positively correlated with NDLQI score(r=0.560,P<0.05).5 Correlation analysis of serum Obestatin expression with NDSI,NDLQI scores before treatment: Serum Obestatin expression was positively correlated with NDSI score(r=0.526,P<0.05),and negatively correlated with NDLQI score(r=-0.514,P<0.05).6 Comparison of serum Ghrelin and Obestatin expression levels between FD group before and after treatment: the serum Ghrelin expression levels of FD group before and after treatment were 4.33±1.70ng/ml and5.61±1.13ng/ml,respectively,and the serum Ghrelin expression level of FD group after treatment was significantly higher than that of FD group before treatment(P<0.05).Serum Obestatin expression levels in FD group before and after treatment were 37.41±7.37ng/ml and 36.74±3.97ng/ml,respectively.Serum Obestatin expression levels in FD group after treatment were lower than those in FD group before treatment,and the difference was not statistically significant(P>0.05).7 Comparison of SAS,SDS,NDSI and NDLQI scores between FD group before and after treatment : SAS,SDS,NDSI and NDLQI scores of FD group before treatment were 54.25±10.22,51.17±11.95,52.25±9.97,54.92±13.71,respectively;SAS,SDS,NDSI and NDLQI scores of FD group after treatment were47.17±4.57,43.58±6.78,45.50±1.17,76.25±8.39,respectively;SAS,SDS and NDLQI scores of FD group after treatment were significantly lower than those of FD group before treatment(P<0.05).NDLQI score in FD group after treatment was significantly higher than that in FD group before treatment(P<0.05).8 Correlation analysis between serum Ghrelin expression level and NDSI,NDLQI scores after treatment: serum Ghrelin expression level was negatively correlated with NDSI score(r=-0.634,P<0.05),and positively correlated with NDLQI score(r=0.580,P<0.05).9 Correlation analysis of serum Obestatin expression with NDSI,NDLQI scores after treatment: Serum Obestatin expression was positively correlated with NDSI score(r=0.516,P<0.05),and negatively correlated with NDLQI score(r=-0.573,P<0.05).Conclusions 1 Compared with healthy people,the expression level of serum Ghrelin in FD patients is decreased,and the expression level of Obestatin is increased.The expression levels of serum Ghrelin and Obestatin are correlated with the symptom degree,quality of life,psychological factor,suggesting that they may be involved in the pathogenesis of FD.2 After mirtazapine treatment,dyspepsia symptoms in FD patients were significantly relieved,which may be related to the increased expression level of serum Ghrelin and indirectly inhibit the negative effect of Obestatin,suggesting that mirtazapine may play a role in the treatment of FD by regulating the expression level of brain intestinal peptide.3Ghrelin and Obestatin may participate in the regulation of
