Observation On The Curative Effect Of Eight Step Needle Knife Therapy On Knee Osteoarthritis Of Strain And Blood Stasis Type

Objective :To observe the effect of eight step needle knife therapy on knee osteoarthritis(KOA)of strain injury and blood stasis type,and to explore the effect of eight step needle knife therapy on clinical symptoms of KOA of strain injury and blood stasis type by using knee circumference,pain VAS score,plantar pressure impulse ratio and WOMAC score.Method:60 patients with knee osteoarthritis were randomly divided into eight step Acupotomy group and electroacupuncture group,30 cases in each group.In the eight step needle knife group,eight sites were selected as the treatment points,including the suprapatellar capsule,the stimulation point of the infrapatellar ligament,the upper and lower part of the medial collateral ligament of the knee joint,the upper and lower part of the lateral collateral ligament,the insertion point of the popliteal muscle and the stimulation point of the medial popliteal fossa.They were treated with eight step needle knife therapy once a week,two weeks as a course of treatment,and one course of observation.In the electroacupuncture group,eight acupoints(yinlingquan,Liangqiu,Xuehai,Yanglingquan,neigengyan,waigenyan,Geshu and Ashi points)were selected.After getting Qi by filiform acupuncture,the electroacupuncture instrument was connected.The waveform was sparse and dense wave(the alternating of sparse and dense waves lasted for about 1.5s each).The needle was retained for 30 minutes,three times a week,two weeks as a course of treatment,and one course of treatment was observed.Knee circumference,plantar pressure impulse ratio,pain VAS score,WOMAC score and curative effect evaluation were used as observation indexes.Before and after treatment,the observation index data of patients were recorded,and the results were discussed after statistical analysis.Achievements:1.There was no significant difference in gender,age and other basic information,knee circumference,plantar pressure impulse ratio,VAS score and WOMAC score between the two groups before treatment(P > 0.05).2.The difference of knee circumference between the two groups before and after treatment was statistically significant by Wilcoxon rank sum test(P < 0.05).The difference of knee circumference between the two groups before and after treatment was statistically significant by Wilcoxon rank sum test(P < 0.05).It shows that the eight step needle knife method and electroacupuncture can reduce the knee circumference,but the eight step needle knife group is better than the electroacupuncture group.3.According to the analysis of variance of repeated measurement of two factors,the pain VAS scores of the patients in the eight step Acupotomy group and the electroacupuncture group decreased after treatment.The pain VAS scores of the two groups were similar before treatment,but there were significant differences between the Babu Acupotomy group and the electroacupuncture group after 1 week,2 weeks and 2weeks of treatment(P < 0.05),indicating that the Babu Acupotomy group alleviated the pain of KOA patients more significantly than the electroacupuncture group.4.After Wilcoxon rank sum test,there was no significant difference in plantar pressure impulse ratio between the two groups before and after treatment(P > 0.05),indicating that there was no significant change in plantar pressure impulse ratio after eight step acupotomy and electroacupuncture treatment.5.According to the analysis of variance of repeated measurement of two factors,WOMAC scores of the two groups decreased,indicating that the eight step needle knife method and electroacupuncture have obvious effect on improving the overall symptoms of patients.The scores of the two groups were similar before treatment and one week after treatment,but there was no significant difference in stiffness scores between the two groups after 2 weeks and 2 weeks after treatment,while there were significant differences in pain score,function score and total score between the eight step needle knife group and the electroacupuncture group after 2 weeks and 2 weeks of treatment(P < 0.05),indicating that the eight step needle knife method and electroacupuncture can improve the stiffness of knee joint The effect was similar.The eight step Acupotomy group was better than the electroacupuncture group in improving pain,functional limitation and overall symptoms.6.After treatment,2 patients were cured,12 patients were markedly effective,14 patients were effective and 2 patients were ineffective in the eight step needle knife group;0 patients were cured,2 patients were markedly effective,24 patients were effective and4 patients were ineffective in the electroacupuncture group.The total effective rate of the eight step needle knife group(93.3%)was higher than that of the electroacupuncture group(86.7%),so it can be considered that the eight step needle knife method is better than the electroacupuncture group in the treatment of KOA.Conclusion:the eight step needle knife method has obvious effect in improving pain,swelling,dysfunction and overall symptoms,and is superior to ordinary electroacupuncture.
