Clinical Study Of Jianzhongchufeng Decoction In The Treatment Of Intractable Allergic Purpura Of Spleen-kidney Yang Deficiency Combined With Liver Stagnation

Objective Observe the clinical efficacy of the instructor’s empirical prescription "Jianzhong chufeng Decoction" in the treatment of "spleen-kidney Yang deficiency combined with liver stagnation type refractory allergic purpura" and its influence on serum cytokine levels of the patients,and monitor the liver and kidney function and ECG to clarify the clinical medication safety,so as to expand new ideas for future research and treatment.Method In this study,patients who were diagnosed as spleen-kidney Yang deficiency combined with liver stagnation type refractory allergic purpura were randomly divided into treatment group and control group,finally with 20 cases in each group.The treatment group was treated with "Jianzhongchufeng Decoction",and the control group was treated with hormones and other symptomatic supportive treatments of medicine.The main clinical symptoms,urine protein,urine occult blood,cytokines(IL-2,IL-4,IL-6,IL-8,IL-10,IL-22,IL-17 A,TNF-α),safety indicators were recorded and statistically analyzed.At the same time,after the treatment,the follow-up record was continued for 4 months and the recurrence was counted.Result1.In terms of the total efficacy of the disease: the total effective rate of the treatment group was 95%,and the control group was 85%,there was no significant statistical difference(P>0.05).2.In terms of improvement in the main clinical manifestations of the disease: the treatment group and the control group had statistical differences in skin purpura,jointpain,abdominal pain,urine routine qualitative indicators(urinary protein,urine occult blood)before and after treatment(P<0.05).After treatment,the treatment group had obvious advantages over the control group in skin purpura and joint pain and was statistically significant(P<0.05),and there was no significant difference in abdominal pain and urine routine(P>0.05).3.In terms of the efficacy of TCM syndromes: the total effective rate of the treatment group is 95%,which was significantly higher than that of control group(65%)(P<0.05).After treatment,the treatment group was significantly better than the control group in the improvement of lumbar debility,chills,chest and hypocholic distension and pain,loose stools,nocturia(P<0.05),there were no significant differences in emotion,lack of food,dark complexion,abdominal distension pain(P>0.05).4.Comparison of recurrence rate 4 months after the end of treatment: the treatment group was 5.26%,and the control group was 35.29%,there was a statistical difference(P<0.05).5.After treatment,IL-4,IL-6,IL-8,IL-10,TNF-α,IL-17 A and IL-22 in the treatment group were significantly decreased(P<0.05),while IL-2 was significantly increased(P<0.05).After treatment,IL-4,IL-6,IL-8 and IL-10 in the treatment group were significantly lower than the control group(P<0.05),IL-2 was significantly increased(P<0.05),TNF-α,IL-17 A and IL-22 groups had no statistical significance(P>0.05).6.In terms of safety indicators: There were no significant differences in safety indexes(AST,ALT,BUN,SCR,ECG)between the two groups before and after treatment(P >0.05).Conclusion The tutor’s experience "Jianzhongchufeng Decoction" has significant clinical efficacy and safety in the treatment of refractory allergic purpura,the recurrence rate is significantly lower than that of the control group,and it can significantly improve urine protein,urine occult blood,and cytokines.
