Background:Total knee arthroplasty(TKA)is a successful treatment for end-stage knee osteoarthropathy.The important evaluation criterion of this operation is the full-length DR of the lower limb after surgery.The imaging results can be used to measure the lower limb mechanical axis(MA).When using conventional techniques for TKA,the surgeon rely on bony landmarks for positioning and alignment.In some special patients undergoing TKA,such as obesity,severe joint deformity,it is difficult for conventional techniques to place the prosthesis in the correct position and cannot obtain a satisfactory MA.Therefore,people invented navigation system.Traditional navigation requires a larger operating console.The surgeon needs to insert the guide pin on the femur and tibia for positioning and registration before the operation can be performed.Although more accurate lower limb force lines can be obtained,the operation time is longer.T portable navigation was invented,and iASSIST navigation system is one of them.The navigation system mainly uses gyroscopes and accelerators for sensing,and is connected to the console through bluetooth module.The surgeon only needs to use 2 pods for osteotomy guidance and verification of the osteotomy.Methods:This article is a retrospective study.A total of 48 patients undergoing TKA in our department in 2016 were collected.Among them,the iASSIST navigation system is the navigation group,and the conventional technology is the conventional group.Paired t test was used to analyze the preoperative and postoperative MA angles and WOMAC knee scores in the two groups.The independent t test was used to analyze the MA angle between the two groups,the WOMAC knee score and FJS joint forgetting score at 2 years postoperatively;the WOMAC knee joint score and FJS joint forgetting score at 5 years postoperatively.Results:A total of 23 people in the navigation group,with 34 knees,and 25 people in the traditional group,with 25 knees were included.Postoperative MA and WOMAC were significantly improved in both groups(p<0.0001).The WOMAC score of the navigation group was 16.2±5.8 at 2 years postoperatively,20.1±5.7 points in the traditional group,p=0.013;the WOMAC score in the navigation group was 14.5±4.6 at 5 years postoperatively,and 17.7±5.3 in the traditional group,p=0.019.The FJS score of the navigation group was 81.5±8.84 points after 2 years after the operation,and the traditional group was 74.2±8.7 points,p=0.01;the average FJS score of the navigation group was 83.6±6.9,and the average FJS score of the traditional group was 80.0±5.0.p=0.046.Conclusion:The iASSIST navigation system can improve the accuracy of prosthesis placement,better reset the lower limb force line,and obtain better and excellent mid-term clinical effects.
