Objective: To observe the changes of serum complement level in children with Kawasaki disease(KD)and juvenile idiopathic arthritis(So-JIA),and to explore its significance in differential diagnosis and prognosis of the two in clinical work.Methods:A retrospective analysis was conducted on the children with KD disease and So-JIA disease treated in our hospital in the past 10 years.Finally,40 children in each group meeting the inclusion criteria were selected,and 40 healthy children in physical examination were randomly selected as the control group.(1)The peripheral blood of children with KD and So-JIA disease was collected.The blood routine,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and serum ferritin(SF)levels of each sample were detected.(2)The levels of complement C3 and complement C4 in peripheral blood of all subjects were detected,and the measured indexes were statistically analyzed.Results:The study found that the changes of leukocyte neutrophils,platelets and hemoglobin were consistent between the two groups,and there was no statistical difference(P>0.05);The level of CRP and ESR SF in So-JIA group was significantly higher than that in KD group(P<0.05);Serum complement C3 and C4 levels in both groups were higher than those in the control group(P<0.05),and the level of complement C3 in the So-JIA group was significantly higher than that in the KD group(P<0.05),complement C3 was negatively correlated with serum ferritin in So-JIA group(r=-0.312,P =0.049);In the KD group,there was a positive correlation between complement C3 and ESR(r=0.482,P =0.002).According to the ROC curve of clinical significance and disease diagnosis level,the areas of SF、CRP、ESR 、C3 and C4 under the ROC curve of So-JIA were 0.864、 0.728 、0.689、0.814、0.689,respectively,indicating that all research indicators have certain value in the diagnosis of So-JIA,among which SF and complement C3 have the most significant diagnostic value.Conclusion: In this study,it was found that in the statistical analysis of ESR、CRP、SF、complement C3 and complement C4,the level of So-JIA group was higher than that of KD group,and the area under the ROC curve also indicated the value of each indicator in the diagnosis of So-JIA,which could be used clinically in combination.In the two diseases,there was a significant correlation between complement C3 and serum ferritin KD、erythrocyte sedimentation rate in the So-JIA group,respectively,which provides important value for the study of pathogenesis and disease monitoring.
