Clinical Insights Of Domestic And Imported Mesh And Suspender In The Treatment Of Anterior Vaginal Wall Prolapse And Stress Urinary Incontinence

Objective To compare the clinical effect of domestic and imported mesh and sling in the treatment of anterior vaginal wall prolapse and pressure urinary incontinence,evaluate the clinical value of anterior vaginal wall prolapse and TVT-O with sling in the treatment of pressure urinary incontinence by using mesh plate,and make it possible for the national production net piece and sling to replace imported mesh and sling.Methods 70 patients were diagnosed as prolapse of anterior wall of vagina and incontinence of pressure urinary tract from March 2014 to June 2015.37 patients were treated with net plate for anterior pelvic reconstruction(group A),30 patients underwent transobturator vaginal tension free middle segment suspension(TVT-O)(group B),and randomly divided the two groups into experimental group and control group.In group A,18 were used in the operation of the experimental group,19 were used in the control group,and15 were used in group B,15 were used in the control group.All the patients were qualified chief doctors.There was no difference in the general data and preoperative quality of life score between the two groups,which was comparable.The results showed that the patients in the two groups had the symptoms of prolapse,leakage and frequency,frequency,urgency,operation time,bleeding and complications,hospitalization time,postoperative hospitalization time and time of removing catheter,and the short-term effect of the operation was evaluated;The quality of life of group A was assessed by pfiq-7 and pfdi-20.The quality of life of group B was evaluated by iqol and ici-q-sf.The quality of life scores of the experimental group and the control group were compared before and after operation;The recurrence rate and complications of the two groups were compared and analyzed in 3 months,1 year,3 years and5 years after operation to evaluate the long-term clinical effect.Results 1.There was no significant difference between the experimental group and the control group in age,marriage age,pregnancy time,labor time,operation history,complications,prolapse of anterior vaginal wall and quality of life before operation(P > 0.05);There was no significant difference in the length of hospitalization,operation time,the degree of prolapse of anterior wall of vagina between the two groups(P > 0.05);There was no significant difference in the quality of life score between the two groups in 5 years after operation(P > 0.05),and there was significant difference between the five years after operation and the preoperative quality of life score(P < 0.05);There was no significant difference in recurrence rate between the two groups in 5 years after operation,and there was no significant difference in the complications of mesh exposure and pelvic pain between the two groups(P > 0.05).2.In group B,the experimental group and the control group were in the age,marriage age,pregnancy,birth,operation history,complications and preoperative There was no statistical difference in the quality of life questionnaire scores(P > 0.05),and the two groups were comparable;There was no significant difference between the two groups in hospitalization time,intraoperative bleeding,operation time,preoperative,postoperative leakage and frequency,frequency of urine,and symptoms of urgency(P > 0.05);There was no significant difference in the quality of life score between the two groups in 5 years(P >0.05),and there was significant difference between the five years after operation and the preoperative one(P < 0.05);There was no significant difference in recurrence rate between the two groups(P > 0.05);There was no significant difference between the two groups in the complications of sling exposure and pelvic pain(P > 0.05).Conclusion 1.There is no difference in recurrence rate and complications between domestic mesh and imported mesh in the treatment of severe anterior vaginal prolapse.2.The clinical effect of TVT-O with domestic sling in the treatment of moderate and severe stress urinary incontinence is exact,and there is no difference in recurrence rate and complications between domestic sling and imported sling.
