ObjectiveFebrile Seizure is one of the most common convulsive diseases in children caused by fever in childhood.Although febrile convulsion is an age-limited disease,febrile convulsions do harm to children,their families and social resources in varying degrees.Materials and methodsThis study is a multicenter,open and forward-looking "real world study",led by the second affiliated Hospital of Medical College of Shantou University.In conjunction with12 hospitals,114 children diagnosed with febrile convulsion were followed up and observed.A total of 388 cases of recurrent fever occurred.All the children in the group were divided into two groups according to the wishes of their families.Group A was the control group(without preventive drugs,only antipyretic and symptomatic treatment),B group was treated with diazepam prophylaxis at the same time of antipyretic and symptomatic treatment(0.3mg/(kg.time).The maximum dose was no more than 10 mg,once every 8 hours,≤ 3 times a day,when fever occurred for 3 days);C group was treated with levetiracetam prophylaxis(7.5-15mg/(kg.time),once every 12 hours,after continuous administration for 3 days)in addition to antipyretic and symptomatic treatment.The fever,prevention methods and morbidity of children were followed up regularly,and the effects of different drugs on short-term prevention of febrile convulsion in children were studied and analyzed.Result1.A total of 114 children were enrolled in this study,of which 96 completed the study.A total of 388 cases of fever,Among them,there were 235 cases of febrile convulsion in group A,95 cases of recurrent febrile convulsion,63 cases of febrile convulsion in group B,7 cases of febrile convulsion and 90 cases of febrile convulsion in group C.among them,there were 1case of recurrent febrile convulsion.There were significant differences between group A,group B and group C(P ≤0.05),In order to prevent the recurrence of febrile convulsions,group C is the best,group B is the second,and group An is the worst.2.The median age of the first attack in the group was 23 months,which coincided with the peak age of the first attack in the research progress of febrile convulsion.Among them,27 cases were treated with drug prophylaxis immediately after the first attack.There were 2 cases of convulsion(7.41%).A total of 113 cases were treated with basic antipyretic and symptomatic treatment,of which 48 cases had secondary convulsions(42.48%).Among the children who were enrolled in the group after non-first attack,there were 126 cases of prevention,6 cases of recurrent convulsion(4.76%).122 cases of basic antipyretic and symptomatic treatment,and 47 cases of recurrent convulsion(38.52%).There was significant difference between group A(blank control group)and group B and C(total children with drug prophylaxis)(P ≤ 0.05).That is,whether or not there are children who begin to take medication to prevent febrile convulsion after the first attack,the recurrence rate of febrile convulsion after medication is lower than that of simple antipyretic and symptomatic treatment.Conclusion:1.In the prevention of recurrent febrile convulsions,the preventive effect of levetiracetam solution was better than that of diazepam tablets,and the preventive effect of levetiracetam solution was better than that of the blank control group,while the preventive effect of diazepam tablets was better than that of the blank control group.2.The probability of recurrent convulsion after fever is still high,no matter whether it is the first attack or the second or multiple febrile convulsions,and the recurrence rate of febrile convulsions treated with drug prophylaxis is lower than that of simple antipyretic and control treatment,so it is necessary to take drug prophylaxis for all children with febrile convulsions.
