A Study About Prediction Of P300 In Violent Behavior Of Schizophrenic Patients

Objective To probe into the change of event related potential P300 in schizophrenia patients with diverse levels of violence risk,to compare difference with or without violent behavior,and to analyze the role of P300 in predicting incident of violence.MethodsSchizophrenic patients from a psychiatric hospital in Shihezi,named as Lvzhou Hospital,from January 2019 to August 2020 were recruited for this study,and the risk of violence was assessed for 7 days.According to the assessment results,the patients who were up to standard the inclusion criteria were segmented into low-risk group,medium risk group and high-risk group.The patients in each group completed the auditory P300 test within3 days,and the patients were observed whether the violence occurred within 3 days after the P300.Patients was classified into two groups,one with violence and other without violent according to whether it was occurred or not.Results1.Significant difference has not been discovered in P300 latency among the three groups(P > 0.05),but the difference of amplitude was statistically significant(F = 6.671,P< 0.05).After pairwise comparison,significant difference has been discovered in P300 amplitude between low risk group and high risk group,medium risk group and high risk group,but significant difference has not been discovered between low risk group and medium risk group;2.There were obvious differences in latency and amplitude between group with violence and group with no violence(F =-7.208,P < 0.001;F = 4.642,P < 0.001);there were evident differences in P300 amplitude between patients with violence and patients without violence(F =-7.208,P< 0.001;F = 4.642,P < 0.001)952,P= 0.004),but significant difference has been discovered in latency(t =-1.369,P> 0.05).3.The receiver operating characteristic curve(ROC)showed that the area under the curve(AUC)of P300 with latency and amplitude was 0.762,whose sensitivity was 54.1%,whose specificity was 91.9%,whose false positive rate was 6.6%,whose false negative rate was 45.9%,and the Youden index was 0.475.Compared with the latency AUC = 0.762,Z = 3.798,P < 0.01;compared with the amplitude AUC = 0.796,Z = 3.19,P < 0.001),and better than the BVC grade(Z = 3.038,P= 0.0024 <0.01);the comprehensive effect of P300 and BVC can improve the prediction efficiency(comprehensive effect AUC= 0.944,Z=2.25,P < 0.05,Z=4.048,compared with the BVC grade AUC).The kappa coefficient of P300 was 0.728,and the reliability was high.ConclusionThe latency and amplitude of P300 can be used as a specific neurophysiological index to evaluate and judge the risk of violence in patients with schizophrenia.It has high sensitivity and specificity for predicting violence in patients with schizophrenia.It can be used as a screening and monitoring tool,and will be widely used in the future evaluation.
