| Objective:To understand the status and influencing factors of self-care level of arteriovenous fistula,and to explore and analyze the relationship between self-care of arteriovenous fistula,coping style and family support,so as to provide theoretical basis and guidance in improving self-care intervention of arteriovenous fistula patients.Methods:Questionnaire survey method: Cluster sampling method was used to conduct a questionnaire survey among 270 maintenance hemodialysis patients from two third-class A hospitals in Shihezi from December 2019 to March 2020.Statistical analysis methods:(1)The general data of maintenance hemodialysis patients were analyzed by descriptive statistics using constituent ratio.(2)The self-care behavior scores,coping style scores and family support scores of maintenance hemodialysis patients with arteriovenous internal fistula were described by means ± standard.(3)Univariate analysis: t test,analysis of variance.(4)Correlation analysis: Spearman analysis.(5)Multi-factor analysis: multiple linear regression.Results:1 Current situation of self-care of arteriovenous fistula: The total score of self-care of arteriovenous fistula was 56.13±10.29,the score of symptom and sign recognition dimension was 23.19±3.99,and the score of complication prevention dimension was 32.94±6.81.2 Coping styles: the score of face factor was 18.79±3.50 points,which was lower than the norm mode,the score of avoidance factor was 15.97±4.79 points,which was lower than the norm,and the score of yield factor was 11.55±4.26 points,which was higher than the norm.3 Family support status: The family support score was 11.21±2.04 points,including 124cases(45.92%)in the high family support group and 146 cases(54.08%)in the low family support group.4 Study on the relationship between self-care of arteriovenous fistula and coping style and family support in maintenance hemodialysis patients:(1)Correlation analysis: The total score of arteriovenous fistula self-care behavior is positively correlated with the face factor(r=0.673,P<0.01),and negatively correlated with the yield factor(r=-0.559,P<0.05),and Family support was positively correlated(r=0.410,P<0.01).(2)Multi-factor analysis of arteriovenous fistula self-care for maintenance hemodialysis patients: face factors(β=0.454),family support(β=0.261)and gender(β=-0.091),education level(β=0.201),the number of fistula operations(β=-0.088),have become the main influencing factors of arteriovenous fistula self-care behavior.The difference was statistically significant(P<0.05),which together explained 53.6% of the total variation.Conclusion:1.Self-care behavior of arteriovenous internal fistula of maintenance hemodialysis patients investigated needs to be improved;Compared with the norm,the coping style of patients is more inclined to adopt the submission coping style and less to adopt the face coping style.In addition,their family support needs to be improved.2.The self-care of arteriovenous fistula in maintenance hemodialysis patients was positively correlated with the face factor and negatively correlated with the yield factor,and positively correlated with family support.Face factor and family support were the main influencing factors for self-care of arteriovenous fistula.Therefore,in the health promotion and education,medical staff should encourage patients to take positive coping ways to treat the disease and devote themselves to strengthening the family support of patients,so as to improve the self-care of arteriovenous internal fistula. |
