| Objective:To retrospectively analyze the clinical datas of patients with chronic heart failure(CHF),to understand the clinical features,clinical complications and etiology of patients with chronic heart failure,and provide a clinical reference for the development of a CHF prevention and treatment program for residents in the region.So that the prevention and treatment of chronic heart failure is more regional,rational and standardized.Methods:By adopting the method of retrospective case analysis,reviewing the medical records database、His medical record system of the Affiliated Hospital of Youjiang Medical University for the treatment of chronic heart failure patients from January 1,2013 to December 31,2018.The collected indicators include:name,gender,age,ethnicity,hospitalization days,smoking history,drinking history,etiology,merger other diseases,cardiac function grading,echocardiograhpic,blood routine,biochemical examination,etc.then enter data and establish a database.According to the level of ejection fraction,it is divided into two groups: heart failure with reduced left ventricular ecjection fraction(group A)and heart failure with preserved left ventricular ecjection fraction(group B).According to age,there are 4 groups: <18 years old,18-40 years old,40-65 years old,≥65 years old.According to the time period,it is divided into two periods: 2013-2015 and 2016-2018..The above case data were statistically analyzed and compared by using the SPSS 20.0 statistical software package.Results:(1)In 4841 cases,2744 cases of male in patients with chronic heart failure(CHF)(56.70%),2097 cases(43.30%)female,there were more male than female(P<0.05).The overall average age was(62.92 ± 13.12)years old,the average age of male patients was(62.26 ± 13.15)years old,the average age of female patients was(63.81 ± 13.02)years old,and the proportion of patients who over 65 years old was the highest.Patients with heart failure withr reduced left ventricular ecjection fraction(group A)were 1061cases(21.92%),patients with heart failure with preserved left ventricular ecjection fraction(group A)were 3780 cases78.08%),the male patients were significantly more than the females in two groups,and the gender composition ratio was significantly different(P<0.05).In general,at the time of admission,the heart function was grade II and grade III,among them,heart function class II accounted for the largest proportion,accounting for 60.40%.(2)Among4841 patients with chronic heart failure,the proportion of total clinical comorbidities was as follows: 2550 cases(52.68%)with hypertension,1821 cases(37.64%)with pulmonary infection,and 1568 cases(32.40%)with arrhythmia.There were 1084 cases(22.39%)of hyperlipidemia and 595 cases of diabetes(12.29%).Among them,the highest proportion of clinical comorbidities in group A was pulmonary infection 591cases(55.7%),followed by 372cases(35.06%)in hypertension.The highest proportion of clinical complications in group B was hypertension 2180 cases(57.67%).),followed by 1230 cases(32.57%)of pulmonary infections.There was a statistically significantly differenct in the distribution of clinical comorbidities between the two groups(P<0.05).(3)4841 cases of chronic heart failure patients mainly include coronary heart disease,rheumatic heart disease,dilated cardiomyopathy,etc.Among them,coronary heart disease is the mainly cause of chronic heart failure patients in this region,and it has been the first cause of the disease.In the two periods(2013-2015 and 2016-2018),the proportion of the patients showed an upward trend(P<0.05);The proportion of dilated cardiomyopathy and cardiomyopathy showed a downward trend(P<0.05).Among male patients,the proportion of coronary heart disease,dilated cardiomyopathy,and pulmonary heart disease is significantly higher than that of female patients;among female patients,the proportion of rheumatic valvular heart disease,hyperthyroid cardiomyopathy,and senile valvular heart disease was significantly higher than that of male patients(P<0.001).(4)Among 4841 patients with chronic heart failure,the main cause of CHF in men who under 40 years old is dilated cardiomyopathy,and the main cause of CHF in women who under 40 years old is rheumatic heart disease;After 40 years old,regardless of gender differences,coronary heart disease is the main cause of chronic heart failure,and with the increase of age,the proportion of coronary heart disease gradually increases and the proportion of dilated cardiomyopathy decreases.Conclusion:(1)Among patients with chronic heart failure in this area,male patients have a higher prevalence than females,among them,the elderly who over 65 years old are the most common,at the time of admission,the heart function of patients with chronic heart failure was mainly grade II and grade III,and the proportion of patients with central function grade II was the highest.(2)In the past 6 years,hypertension,pulmonary infection,and arrhythmias are common clinical complications of chronic heart failure in the area,among them,hypertension is the most common clinical complication of heart failure with preserved left ventricular ecjection fraction;pulmonary infection is the most common clinical complication of heart failure with reduced left ventricular ecjection fraction.(3)In the past 6years,the causes of chronic heart failure in this area mainly include coronary heart disease,rheumatic heart disease,and dilated cardiomyopathy,among them,coronary heart disease is the most common cause,and the proportion is increasing year by year;the proportion of dilated cardiomyopathy is decreasing year by year. |
