Objective: To analyze the correlation between serum 25 hydroxyvitamin D and heart failure and to analyze the clinical characteristics of patients with heart failure in different ejection fraction classifications.Methods: The 427 heart failure patients included were divided into 145 cases of HFrEF,145 cases of HFmrEF,and 137 cases of HFpEF,and the control group was 153 healthy physical examination patients of the same period.The general clinical data,serum 25(OH)D,and echocardiographic correlations were collected to analyze the clinical characteristics of each classification of heart failure patients;Spearman correlation analysis was used to investigate the correlation between serum 25(OH)D and cardiac structural and functional indices and NT-proBNP;logistic regression analysis was used to investigate the effect of serum 25(OH)D The impact of serum 25(OH)D on the classification of heart failure was investigated by logistic regression analysis,and the correlation between serum 25(OH)D and each classification of LVEF were investigated by multiple linear regression analysis;the subject operating characteristic curve(ROC)was plotted,the area under the curve(AUC)was calculated to assess the diagnostic predictive value of serum 25(OH)D assay for the disease,and the cut-off value was calculated.Results: The HFrEF group had predominantly cardiomyopathy,with a higher proportion of cardiac function class III than the other two groups;the HFmrEF group had predominantly coronary heart disease,with a higher incidence of previous angina pectoris and the presence of ventricular wall tumors than the HFpEF group;the HFpEF group had predominantly valvular disease,with a higher incidence of previous atrial fibrillation and a higher incidence of hypertension than the HFrEF group.The serum 25(OH)D level in non-heart failure group was significantly higher than that in heart failure group(P < 0.05),and the serum NT-proBNP level was significantly lower than that in heart failure group(P < 0.05).Spearman correlation analysis showed that serum 25(OH)D was weakly negatively correlated with RVOT,PAP,and NT-proBNP(P < 0.05),and weakly positively correlated with FS,LVEF,and SV(P < 0.05).Logistic regression analysis showed that the serum 25(OH)D level was higher in HFpEF group,and the lower the serum 25(OH)D level,the more likely to develop other types of HF(95%CI 1.003-1.054;P=0.025).Multivariate linear regression analysis showed that LVEDD,IVS,FS,SV,CO had a linear relationship with LVEF in HFrEF group(P < 0.05).There was a linear relationship between LVESD,IVS,LVPW,FS,ACEI/ARB,ARNI and LVEF in HFmrEF group(P < 0.05).There was a linear relationship between serum 25(OH)D,FS,CO,diuretics and LVEF in HFpEF group(P <0.05).Conclusions: Deficiency of serum 25(OH)D was widespread in patients with heart failure and was lowest in the HFrEF group;Serum 25(OH)D levels are positively correlated with LVEF,and their levels may be closely related to systolic function of the left ventricle;Serum 25(OH)D levels may be useful in the classification and diagnosis of patients with heart failure,especially in patients with HFpEF.
