Objective: To explore the clinical characteristics of post-menopausal women with high-grade cervical intraepithelial neoplasia.Methods: Through retrospective analysis,147 postmenopausal patients who were diagnosed with high-grade cervical intraepithelial neoplasia from January 2014 to July 2019 were collected and divided into recurrence groups(n=21),No recurrence group(n=126).By observing the general information(age,pregnancy times,parity,sexual life history),clinical observation indicators(HPV16/18 infection,lesions involving glands,anti-HPV treatment,preoperative and postoperative lesion grades)of the two groups of patients.Through SPSS20.0 software analysis,p<0.05 is statistically significant.Results: Among the 147 postmenopausal patients with high-grade cervical intraepithelial neoplasia,21 cases recurred,and the total recurrence rate was 14.29%(21/147).In the 21 patients with recurrence,9 had CINⅡ(p16+)preoperatively and 12 had CINⅢ.The pathological grades after recurrence were 3 cases of low-grade disease and 18 cases of high-grade disease(8 cases of CINⅡ,10 cases of CINⅢ).Statistical analysis showed that:(1)There was no statistical difference between the age<60 years old group and the age≥60 years old group(p>0.05),suggesting that the age factor is not a characteristic factor related to the recurrence of cervical lesions after surgery.(2)There is no statistically significant comparison between the number of pregnancies <3 times group and the number of pregnancies≥3 times group(p>0.05).The number of pregnancies is not a characteristic factor related to postoperative recurrence of cervical lesions.(3)The parity <2 times group was compared with the ≥ 2 times group with statistical significance(p<0.05),suggesting that prolificacy is a characteristic factor related to the recurrence of cervical lesions after surgery.(4)The postoperative sexual life group and the asexual life group are statistically significant(p<0.05).The postoperative sexual life is a characteristic factor related to the recurrence of cervical lesions.(5)The comparison between the HPV16/18 infection subtype group and the non-HPV16/18 infection subtype group was statistically significant(p<0.05),suggesting that HPV16/18 infection is a characteristic factor related to postoperative recurrence of cervical lesions.(6)The comparison of anti-HPV treatment after operation and no anti-HPV treatment after operation was statistically significant(p<0.05),suggesting that anti-HPV treatment after operation is a characteristic factor related to postoperative recurrence of cervical lesions.(7)The comparison between the glands involved in the lesions and the glands not involved in the lesions is not statistically significant(p>0.05).The glands involved in the lesions are not a characteristic factor related to the recurrence of cervical lesions after surgery.(8)Comparing the preoperative and postoperative lesion grades of CINⅡ(p16+)group and CINⅢgroup respectively,there is no statistical significance(p>0.05),suggesting that the preoperative and postoperative lesion grades are not cervical lesions.Later related characteristic factors.Conclusion: The following conclusions are drawn from the results of this study:(1)The factors related to the recurrence of high-grade cervical intraepithelial neoplasia after menopause are: prolific life,postoperative sexual life history,HPV infection subtype(high-risk type 16/18)、Whether anti-HPV treatment is regularly performed after operation.(2)The factors unrelated to the recurrence characteristics of high-grade cervical intraepithelial neoplasia after menopause are: the age of the patient,the number of pregnancies,whether the pathology involves glands,and the grade of the lesion. |
