BackgroundCirculating tumor cells(CTCs)are closely associated with the formation of colorectal cancer metastases.A number of studies have shown that the number and detection rate of CTCs are closely related to various pathological parameters such as colorectal cancer lymph node metastasis and tumor stage,and tumor cells may undergo epithelial-mesenchymal transformation and produce mesenchymal CTCs and mixed CTCs.However,most previous CTCs detection methods can only detect epithelial CTCs,with certain limitations.The positive detection rate of the representative Cell Search system was less than 50%.In addition,the correlation between the number and detection rate of CTCs and clinicopathological parameters and tumor markers of patients varies in various studies.ObjectiveTo explore the relationship between different cutoff values of epithelial type,mesenchymal type and mixed type CTCs with clinicopathological parameters of colorectal cancer within 7 days after surgery and its significance.MethodA total of 129 patients who received radical resection of colorectal cancer in the Department of Gastrointestinal Surgery,the First Affiliated Hospital of Chongqing Medical University from June 2019 to August 2020 were selected,and none of them received neoadjuvant chemoradiotherapy before surgery.Peripherous venous blood was collected within 7 days after radical resection,and CTCs in peripheral blood were detected by second-generation circulating tumor cells Can Patrol.The correlation between CEA,CA19-9,age,gender,tumor site,postoperative disease examination results and other data was analyzed.ResultsWhen the Cut-off value of various CTCs is ≥2,the detection rate of epithelial CTCs is related to the clinical stage and whether the tumor thrombus is positive or not,while the detection rate of mesenchymal CTCs is mainly related to CA19-9.The mixed CTCs with the highest detection amount is related to the gender and CA19-9 of the patient.When the Cut-off value of various CTCs was ≥3,the detection rate of epithelial CTCs was correlated with the tumor T stage and vascular cancer emboli of the patients,while the detection rate of mesenchymal CTCs was no longer correlated with gender,but still positively correlated with whether CA19-9 was positive or not.The detection rate of mixed type CTCs was high when CA19-9 was negative,but low when CA19-9 was positive,regardless of the CUT-OFF value.In this study,the detection rate and number of the three types of CTCs were not related to the metastasis of tumor lymph nodes or the degree of tumor differentiation in patients.ConclusionsWithin 7 days after surgery,CTCs in peripheral blood were detected to be affected by surgery.The number of CTCs of the three types was not related to clinicopathological parameters and tumor markers of patients.The detection rate was mainly related to tumor stage,vascular tumor thrombus,CA19-9 and other factors,and the relationship changed when different Cut-off values were taken.It is considered that the accuracy of blood sampling in the short term after surgery may be affected by the operation to some extent,and its prognostic value needs to be further studied. |
