Objective:To explore the correlation between vitamin B12 level and anxiety and depression status.Methods:It was a prospective study.The subjects were 117 patients who were first diagnosed with anxiety and depression in the Department of Neurology,the First Affiliated Hospital of Chongqing Medical University.The time range was from February 2020 to December 2020.A total of 141 healthy subjects were included in the control group during the same period.In the first part,baseline data of all subjects were collected.The Hamilton Depression Scale(HAMD)and Zung Self-Rating Depression Scale(SDS),as well as the Hamilton Anxiety Scale(HAMA)and Zung Self-Rating Depression Scale(SAS)were used to evaluate the patients’ status.The levels of vitamin B12 and folic acid were determined by chemiluminescence method.The levels of homocysteine(HCY)were analysed by enzyme analysis.In the second part,117 patients with anxiety and depression were randomly divided into three groups,which were treated with vitamin B12 alone,vitamin B12 and anti-anxiety and depression drugs,anti-anxiety and depression drugs alone for 3 months.The independent sample t-test was used to compare the differences in the level of vitamin B12,folic acid and HCY between the case group and the healthy control group.The paired sample t-test was used to compare the differences in HAMD HAMA SDS SAS scores between each group before and after treatment.The correlation between the level of vitamin B12 and the state of anxiety and depression was used Logistic regression.Results:Compared with healthy controls(390.17±150.81)pg/ml,serum vitamin B12 in patients with anxiety and depression(272.48±145.29)pg/ml were notably lower(P<0.05).The serum homocysteine level in case group(14.91±6.37)μmol/L was higher than that of control group(11.81±10.61)μmol/L(P < 0.05).There was no significant difference in folic acid level(11.07±4.97 ng/ml vs.11.86±5.12 ng/ml)(P>0.05).Logistic regression analysis showed that serum vitamin B12 deficiency(OR 4.0195%CI:1.77-9.12)and high homocysteine(OR 2.38(95%CI:1.33-4.26)were correlated with anxiety and depression status which increase the risk of developing anxiety and depression.As an adjuvant therapy for anxiety and depression,vitamin B12 can notably reduce the anxiety and depression scores of patients with anxiety and depression after treatment(P<0.05),improve the symptoms.Conclusions:Patients with anxiety and depression had a higher proportion of vitamin B12 deficiency,and their serum vitamin B12 level was significantly lower than that of healthy subjects,while their HCY level was higher.There was no significant difference in folic acid level between the two groups.Vitamin B12 deficiency is in connection with anxiety and depression.Vitamin B12 treatment can improve patients’ anxiety and depression symptoms,and combined treatment with anti-anxiety and depression drugs has better efficacy and high safety.
