| Objective:To analyze the relationship between serum NGAL and acute ischemic stroke.The purpose of this study is to explore the correlation between serum NGAL and the severity and short-term prognosis of acute ischemic stroke,so as to guide clinical evaluation of the severity and short-term prognosis of ischemic stroke.Methods:From December 2019 to December 2020,142 patients with acute ischemic stroke who were hospitalized in the Department of Neurology of Shaanxi Provincial People’s hospital were selected as the study group,and 80 patients with non acute ischemic stroke in the same period were selected as the control group.Subgroup analysis was performed among the study groups.The general data,biochemical indexes and imaging data of all subjects were collected.NIHSS score,TOAST classification and 3-month modified rankin score(mRS)were collected at admission in the study group.SPSS26.0 was used for statistical analysis.Results:1.Comparison of general data between study group and control group:there were no significant differences in gender,age,BMI,history of coronary heart disease,history of atrial fibrillation,history of diabetes,history of smoking and history of drinking,but there were significant differences in history of hypertension;2.Comparison of biochemical indexes between study group and control group:there were no significant differences in urea,Cr,TC,TG and LDL-C,but there were significant differences in NGAL,CysC,HCY,HDL-C,WBC and ANC;3.Comparison of different plaque groups and NGAL:the difference of NGAL level in difference plaque groups was statistically significant(F=18.721,P=0.000).There were significant differences in NGAL levels between non plaque group and stable plaque group,non plaque group and vulnerable plaque group,stable plaque group and vulnerable plaque group(P<0.05);4.Comparison of different NIHSS score groups and NGAL:there was significant difference in serum NGAL between mild group and moderate severe group(t=﹣6.853,P<0.05);serum NGAL was positively correlated with NIHSS score(r_s=0.534,P=0.000);5.Comparison of different cerebral infarction volume groups and NGAL:there were significant differences in serum NGAL among three groups(F=55.417,P<0.05);serum NGAL was positively correlated with infarct volume(r_s=0.171,P=0.042);6.Comparison of different mRS score groups and NGAL:there was significant differences in serum NGAL between good prognosis group and poor prognosis group(t=﹣11.619,P<0.05);serum NGAL was positively correlated with mRS scores(r_s=0.742,P=0.000);7.Different Toast classification and NGAL:there was no significant difference in serum NGAL between LAA group and CE group(P=0.583),but there was significant differences in NGAL levels between LAA group and SAO group,CE group and SAO group(P<0.05);8.Binary logistic regression analysis:hypertension(B=1.148,OR=3.151,P=0.002)、NGAL(B=0.024,OR=1.024,P=0.000)、CysC(B=1.914,OR=6.782,P=0.045)and ischemic stroke have statistical significance;9.ROC curve analysis:the area under curve of CysC was 0.692,and when the CysC level was 0.845mg/L,the maximum youden index was 0.293.At this time,the sensitivity and specificity of CysC in the diagnosis of acute ischemic stroke were 76.8%and 52.5%,indicating that the accuracy of CysC in the diagnosis of ischemic stroke was low;the area under curve of NGAL was 0.799,and when the NGAL level was 121.3ng/ml,the maximum youden index was 0.521.The sensitivity and specificity of NGAL in the diagnosis of acute ischemic stroke were 68.3%and 83.8%,indicating that the accuracy of NGAL in the diagnosis of ischemic stroke was moderate.This suggests that the accuracy of serum NGAL in the diagnosis of ischemic stroke was better than CysC.Conclusion:1.Serum NGAL,CysC and hypertension are risk factors of acute ischemic stroke;2.Serum NGAL has a certain value in the diagnosis of acute ischemic stroke;3.Serum NGAL can be used as an index to judge the severity and short-term prognosis of acute ischemic stroke. |
