Objective:Geriatric lung cancer patients are the largest group of patients with tumors,which is easy to suffer distress by chemotherapy,the main treatment of lung cancer.What is more,distress influences the control of the disease and even leads to the occurrence of complications,which is unfavorable for the treatment of lung cancer.This study mainly evaluates the distress of geriatric patients with lung cancer after chemotherapy and exploring the related influencing factors,finding suitable intervention measures,and provides a basis for improving the quality of life of geriatric patients with lung cancer undergoing chemotherapy.Methods:Collected 73 geriatric patients(age ≥ 60 years old)From May 2020 to October2020,who were diagnosed with lung cancer in the Department of Respiratory Medicine of a third-class hospital and received chemotherapy were collected as the experimental group.During the same period,57 geriatric inpatients(age ≥ 60 years old)who were diagnosed with lung cancer in the respiratory department of the same hospital and used targeted therapy served as the control group 1,and 61 community healthy geriatric(age ≥ 60 years)served as the control group 2.Collect general information such as age,gender,type of lung cancer,education level,treatment methods,and assessment results of DT,HAMA,GDS-15,and LSIB.The 73 patients in the chemotherapy group with distress were selected for intervention,and music therapy was used to intervene.The music of the style preferred by the patients was selected as the intervention music,which was performed 3 times a week,30 minutes each time,and reassessed after 1 month.Statistical method: Use SPSS 20.0 statistical software to analyze and process all the statistical data of the included cases: the measurement data are expressed as mean + standard deviation,the difference between the measurement data of the normal distribution is compared with the t test,and the difference between the measurement data of the non-normal distribution is used.Differences between measurement data groups were compared by rank sum test;count data were expressed as percentages.Logistic regression analysis was used to analyze the related influencing factors of distress in geriatric patients with lung cancer after chemotherapy.(P<0.05 indicates that there is statistical significance among the compared groups.)Results:1.General information: There is no significant difference in gender,age,education level,etc.among the groups(P<0.05).2.The statistical analysis of the distress of each group through the DT scale showed that the incidence of distress in the geriatric lung cancer chemotherapy group was76.7%,the incidence of mental distress in the geriatric lung cancer targeted drug group was 63.1%,and that of the healthy geriatrics in the community.The incidence of distress was 27.8%,and the difference between the groups was statistically significant(P<0.05).3.The statistical analysis of the anxiety evaluation of each group through the HAMA scale shows that the anxiety incidence of the geriatric lung cancer chemotherapy group is 63.0%,the anxiety incidence of the geriatric lung cancer targeted drug group is 26.3%,and the anxiety incidence of the healthy geriatrics in the community It was 19.6%,and the difference between the groups was statistically significant(P<0.05).4.The statistical analysis of depression in each group based on the GDS-15 scale showed that the incidence of depression in the geriatric lung cancer chemotherapy group was 46.6%,the incidence of depression in the geriatric lung cancer targeted drug group was 33.3%,and the depression in the healthy geriatrics in the community The incidence rate was 14.7%,and the difference between the groups was statistically significant(P<0.05).5.The statistical analysis of life satisfaction evaluation through the LSIB scale shows that the incidence of life dissatisfaction in the geriatric lung cancer chemotherapy group is 42.5%,the incidence of life dissatisfaction in the geriatric lung cancer targeted drug group is 24.6%,and the community healthy geriatrics.The incidence of life dissatisfaction was 18.0%,and the difference between the groups was statistically significant(P<0.05).6.Multivariate logistic regression analysis showed that fatigue,gastrointestinal reactions,hair loss,frequent urination,chest cavity perfusion,bronchial artery perfusion may be independent risk factors for distress.7.There were statistically significant differences before and after music therapy intervention for pain,insomnia,distress,anxiety(P<0.05),but no statistically significant differences before and after music therapy intervention for fatigue,depression,life satisfaction(P>0.05).Conclusion:1.Geriatric patients with lung cancer are more likely to suffer from distress,anxiety,depression,and unsatisfactory life than the healthy geriatric in the community;Geriatric patients with lung cancer chemotherapy are more likely to suffer from distress,anxiety,depression,and unsatisfactory life than geriatric lung cancer targeted drug users.2.Fatigue,gastrointestinal reactions,pain,insomnia,hair loss,frequent urination,chest cavity perfusion,bronchial artery perfusion may be the independent risk factors for distress.3.Music therapy intervention has a certain improvement in pain,insomnia,distress,and anxiety,but has no obvious improvement in fatigue,depression,and dissatisfaction with life.
