Clinical Observation Of Chinese And Western Medicine Triple Therapy In The Treatment Of Facial Localized Stable Vitiligo

Objective:To investigate the efficacy and safety of fire needle combined with308 nm excimer light and 0.03% tacrolimus ointment in the treatment of facial localized stable vitiligo,in order to provide an effective treatment for vitiligo.Methods:60 patients with facial localized stable vitiligo were randomly divided into experimental group and control group by random number table method.Group A was the control group,and group B was the experimental group,with30 cases in each group.Group A was treated with 308 nm excimer light twice a week and 0.03% tacrolimus ointment twice a day.Group B was treated with fire needle therapy once a week on the basis of group A.The two groups were treated for 12 weeks.The area of naked eye and dermoscope white spots and the incidence of adverse reactions were recored before treatment,at the 4th(T1),8th(T2)and 12 th week(T3)of treatment,respectively,and the repigmentation rate,curative effect and repigmentation mode of the two treatment methods were compared.Dermatologists recorded the dermatological quality of life index(DLQI)and evaluated the clinical efficacy and adverse reactions.Results:1 Baseline Balance Comparison: there was no significant difference in gender,course of disease and age between the two groups(P>0.05).2 Under the macroscopic observation two groups of repigmentation rate:T1,T2,T3,the difference was statistically significant(P<0.001).The repigmentation rate of group B was higher than group A.3 Under the macroscopic observation the two groups of the therapeutic effects at different time points,there was no significant difference between the two groups at T1(P>0.05),but there was significant difference at T2 and T3(P<0.05).The curative effect of group B was better than that of group A.4 Under macroscopic observation comparison of leukoplakia area at same time point,T1,the difference was not statistically significant(P>0.05);T2,T3,the difference was statistically significant(P<0.05).Comparison of leukoplakia area at different time points(T1,T2,T3)between the two groups,the difference was statistically significant(P<0.001).The leukoplakia area in group B decreased faster with time.5 Under the macroscopic observation two groups of repigmentation rate:T1,T2,T3,the difference was statistically significant(P<0.001).The repigmentation rate of group B was higher than group A.6 Under the dermoscopic observation the two groups of the therapeutic effects at different time points,there was significant difference at T1,T2 and T3(P< 0.05).The curative effect of group B was better than that of group A.7 Under the dermoscopic observation comparison of leukoplakia area at same time point,T1,the difference was not statistically significant(P>0.05);T2,T3,the difference was statistically significant(P<0.05).Comparison of leukoplakia area at different time points(T1,T2,T3)between the two groups,the difference was statistically significant(P<0.001).The leukoplakia area in group B decreased faster with time.8 Comparison of DLQI scores at same time point,the difference was statistically significant(P<0.05).Comparison of DLQI scores at different time points(T1,T2,T3)between the two groups,the difference was statistically significant(P<0.001).DLQI scores in group B decreased faster with time.9 Comparison of adverse reactions between the two groups: the difference was not statistically significant(P>0.05).10 Comparison of two color reproduction modes: the difference was not statistically significant(P>0.05).Conclusions:1 Fired needle combined with 308 nm excimer light and 0.03% tacrolimus ointment has a good effect on facial localized stable vitiligo.2 Pigmentation is produced in the acupuncture site of the fire needle,and the fire needle therapy can stimulate local melanin production in the skin.3 Fire acupuncture therapy did not increase the incidence of adverse reactions.4 Early curative effect can be seen under dermoscopy,which is more advantageous in the judgment of curative effect.
