Objective:To investigate the clinical value of lipid metabolism disorder in patients with colorectal cancer by analyzing the lipid metabolism level and clinicopathological characteristics of colorectal cancer(CRC),and to provide theoretical reference for improving living quantity and prognosis of colorectal cancer patients.Methods:240 patients admitted to Affiliated Hospital of Chengde Medical College from October 2018 to October 2020 were selected as subjects,including 80 patients in the observation group(colorectal cancer group),31 males and 49 females,with an average age of(57.11 ± 6.72)years;the control group was divided into two groups: 80 patients in the control group I(colorectal adenoma group),39 males The average age was(57.04 ± 7.08)years;the control group II(healthy group)included 80 healthy persons,30 males and 50 females,with an average age of(54.73 ± 6.67)years.After strict screening,the three groups of patients met the research standards,and the blood lipid indexes including total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein cholesterol(LDL-C)were determined by biochemical analyzer,LDL-C,apolipoprotein A I(Apo AI),apolipoprotein B(Apo B)and serum lipoprotein a(Lpa)levels.To analyze: 1.the differences in lipid levels between the groups.2.the correlation between the lipid metabolism levels of patients in the colorectal cancer group and their clinicopathological characteristics.3.the dyslipidemia of patients in the colorectal adenoma group with different pathological types,and to explore the correlation between hyperlipidemia and colorectal adenoma.4.the lipid metabolism disorder of colorectal cancer patients receiving postoperative capecitabine chemotherapy was investigated to explore the effect of capecitabine on blood lipid of colorectal cancer patients.Results:1 Comparison of blood lipid levels in each group:(1)Comparison of TC levels: the serum TC levels of patients in the colorectal cancer group and colorectal adenoma group were lower than those in the healthy group,and the diversity was statistically significant(P<0.05).(2)Comparison of TG levels:The serum TG levels of patients in the colorectal cancer group were higher than those in the colorectal adenoma group and the healthy group,and the diversity was statistically significant(P<0.05).(3)Comparison of HDL-C and Apo AⅠ levels: The serum HDL-C and Apo AⅠof patients in the colorectal cancer group and colorectal adenoma group were lower than those in the colorectal adenoma group and normal group,and the diversity was statistically significant(P < 0.05).(4)There was no statistically significant diversity between the three groups when comparing the serum LDL-C,Apo B and Lpa levels of patients in the three groups(P>0.05).2 The relationship between clinical features and blood lipids in colorectal cancer group:(1)gender: the serum TG level of female patients with colorectal cancer(49 cases)were higher than those of male patients(31 cases),the differences were statistically significant(all P<0.05).(2)Tumor location:the serum TC,TG and LDL-C levels of 38 patients with right colon cancer were lower than those of 42 patients with left colon and rectal cancer(all P<0.05).(3)Tumor infiltration depth: 9 cases in T1 group,46 cases in T2 group,27 cases in T3 group and 18 cases in T4 group.The serum TC level in T2 group was lower than that in T1 group,and that in T3 group was lower than that in T2 group,with statistically significant differences(P < 0.05).The serum HDL-C level in T2 group was lower than that in T1 group,and that in T3 group was lower than that in T2 group,with statistically significant differences(P<0.05).(4)TNM staging : 10 cases in phase I group,26 cases in phase II group,27 cases in phase III group,and 17 cases in phase IV group.The serum TC level in phase III group was lower than that in phase II group,and that in phase IV group was lower than that in phase III group,with statistically significant differences(P < 0.05).The serum HDL-C level in phase II group was lower than that in phase I group,and that in phase III group was lower than that in phase II group,with statistically significant differences(P<0.05).The serum Apo A I level in phase II group was lower than that in phase I group,and that in phase III group was lower than that in phase II group,with statistically significant differences(P<0.05).There were no statistical differences in the levels of the indicators of lipid metabolism compared with age,vascular infiltration and histological type(P>0.05).3 Differences in blood lipid levels among patients with different pathological types of colorectal adenoma : in the 80 colorectal adenoma group,there were 50 cases of tubular adenoma,22 cases of tubular villous adenoma and 8 cases of villous adenoma.The serum TC level in villous adenoma group was higher than that in tubular adenoma group and villous-tubular adenoma group,and the diversity was statistically significant(P<0.05).4 Effect of capecitabine on blood lipid level of patients with colorectal cancer: the serum TG,HDL-C,LDL-C levels of patients with colorectal cancer receiving capecitabine chemotherapy were higher than those before chemotherapy,the differences were statistically significant(P<0.05).Conclusion:1 The metabolic changes of serum triglyceride(TG),high density lipoprotein cholesterol(HDL-C)and apolipoprotein A1(Apo AⅠ)may be closely related to the occurrence and development of colorectal cancer.2 With the deepening of colorectal tumor invasion,TC and HDL-C levels showed a downward trend;abnormal metabolism of TC,apo a Ⅰ,and HDL-C was associated with the depth of invasion and TNM stage of colorectal cancer,and preoperative detection of their levels was helpful for disease assessment and prognosis judgment.3 There is a positive correlation between the increase of serum total cholesterol(TC)and the increase of villus structure of adenoma(or the increase of canceration rate of adenoma),suggesting that patients with high serum TC level have an increased chance of canceration.4 Capecitabine can adversely affect lipid metabolism in patients with postoperative chemotherapy for colorectal cancer,characterized by elevated serum TG,HDL-C and LDL-C.Lipid monitoring and effective prevention should be carried out in patients with colorectal cancer taking capecitabine to avoid clinical adverse events.
