Background With the development of society,changes in dietary structure,lifestyle and living environment,cardiovascular disease has become a major disease that seriously threatens human life and health.Ischemic heart disease,also known as coronary heart disease,is the most common cardiovascular disease.As is known to all,diabetes is an equal-risk disease of ischemic heart disease.Patients with diabetes combined with ischemic heart disease have more coronary plaques,more diffuse lesion sites and more complex conditions.Diabetic ischemic heart disease has become the main cause of death in diabetic patients.It can be seen that the prevention and treatment of diabetic ischemic heart disease has reached the point of no delay.Objective Recent studies have found that GPR43,as a metabolite receptor,plays an important role in many metabolic processes in the body,affecting the occurrence and development of a series of metabolic diseases such as diabetes and ischemic heart disease.Based on this,this study will compare the expression of GPR43 in monocytes and the concentrations of NLRP3,IL-1β and IL-18 in plasma of patients in different case groups to explore the correlation between GPR43 and diabetic ischemic heart disease.Methods(1)Among the patients undergoing coronary angiography,50 patients with simple diabetes and 61 patients with diabetes complicated with ischemic heart disease were included as the study group according to the inclusion and exclusion criteria;59 patients without diabetes or ischemic heart disease were selected as the control group.Peripheral venous blood mononuclear cells were collected from patients,and the expression of GPR43 in patients of the three groups was detected by Western Blot and q RT-PCR from the protein level and m RNA level,respectively.In addition,venous plasma of patients in the three groups was collected,and the contents of NLRP3,IL-1β and IL-18 in plasma of patients in the three groups were determined by ELISA.The expression of GPR43 and the contents of NLRP3,IL-1β and IL-18 in serum of the three groups were compared,and the correlation between the expression of GPR43 protein and NLRP3 and its downstream inflammatory factors IL-1β and IL-18 in each group was analyzed,so as to evaluate whether GPR43 was correlated with diabetic ischemic heart disease.(2)According to the Gensini score in patients with diabetes,ischemic heart disease can be divided into mild lesions subgroups and severe disease group,compared two groups GPR43 expression and serum NLRP3,IL-1β and IL-18 concentration,and analyze the two groups of patients with inflammatory markers in the expression,NLRP3 and downstream GPR43,IL-1β and IL-18 and the relationship between coronary artery lesion severity,assess whether GPR43 degree of ischemic heart disease and diabetes coronary lesions.Results(1)Compared with normal control group,the expression level of GPR43 in peripheral blood monocytes and the contents of NLRP3,IL-1β and IL-18 in plasma were significantly increased in diabetic group and diabetic ischemic heart disease group,and the expression level of GPR43,NLRP3,IL-1β and IL-18 in diabetic ischemic heart disease group were also significantly higher than those in diabetic group(P<0.05);The expression level of GPR43 protein was positively correlated with the concentrations of NLRP3,IL-1β and IL-18 in 3 groups(P<0.0001,the correlation coefficients are r=0.619,r=0.684,r=0.694).(2)Compared with the mild coronary artery disease subgroup,the expression level of GPR43 in peripheral blood monocytes and the level of NLRP3,IL-1β and IL-18 in plasma in the severe coronary artery disease subgroup were significantly increased(P<0.05);The Ginsini score of the two subgroups was positively correlated with the expression of GPR43 protein and the concentrations of NLRP3,IL-1β and IL-18(P<0.0001,the correlation coefficients were r=0.543,r=0.519,r=0.602,r=637).Conclusion(1)GPR43 is correlated with the occurrence and development of diabetic ischemic heart disease,and the expression level of GPR43 protein is increased in patients with diabetic ischemic heart disease.(2)GPR43 was correlated with the degree of coronary artery disease.The higher the expression level of GPR43 protein was,the more severe the degree of coronary artery disease was in patients with diabetic ischemic heart disease.
