Study On Instant Effect Of Head Needle And Synchronous Rehabilitation Of Swallowing Failure After Stroke

Objective:Ultrasound was used to observe the immediate effect of hyoid bone displacement in patients with post-stroke dysphagia(women),and to compare the efficacy of scalp acupuncture synchronous rehabilitation training and post-head acupuncture rehabilitation training in the treatment of post-stroke dysphagia.To explore the relationship between the synergistic effect of scalp acupuncture synchronous rehabilitation training and the immediate effect of scalp acupuncture,and to provide the basis for clinical use of scalp acupuncture synchronous rehabilitation training.Methods: Experiment 1: the number and scale of the patients who met the inclusion criteria were evaluated and recorded on the day of visit or admission,and the hyoid displacement and record were observed by ultrasound when the patients drank 5 ml of water before scalp acupuncture.～30.When the resistance of the sensory finger is reduced,the needle can be stopped,indicating that the needle tip reaches the lower cap aponeurosis,the frontal midline needle direction is the court direction,the parietal median line is 100 will be forward,the parietal frontotemporal and parietal posterior oblique line is downward from the starting point,twirling needle at about 200 rpm,needle 2～3 min,after the needle is finished;the lingual bone displacement of patients drinking5 ml water is measured again and recorded in the same way.Analyze and discuss data.Experiment 2: the number of 60 patients who completed experiment 1and the evaluation results of the scale were recorded.The patients were randomly divided into experimental group and control group,30 cases in each group.The two groups received the same basic treatment at the same time,the experimental group patients with scalp acupuncture for rehabilitation training,the control group in the morning head acupuncture treatment,afternoon rehabilitation training.Operation content: head needle: take appropriate position,choose bilateral topotemporal front and posterior oblique line 2/5,top midline and frontal midline needle,needle tip and scalp 15.～30.Into,Stop the needle when the resistance drops,indicating that the tip of the needle reaches the lower cap aponeurosis,The middle line of the forehead is in the direction of the court to the printing hall,The center line of the top is 100 will go forward,The anterior and posterior parietal temporal oblique lines are downward from the starting point,Twist the needle at about 200 rpm,Each line of the needle was min 15 and pulled once,Two ～ three min,each30 min;total A total of 30 min: of rehabilitation,neck relaxation,Swallowing training includes jaw movement,lip movement,tongue movement,Sucking training,Mendelssohn,Cold stimulation of the pharynx,Breathing training.Head acupuncture and rehabilitation training once a day,Six times a week,For 2 weeks,A total of 2 courses of treatment.Observation contents: drinking water test,swallowing function evaluation scale(SSA)and functional oral food intake scale(FOIS).Analyze the data and discuss it.Results:Experiment 1:1.There was a significant difference in hyoid displacement between patients(women)before and after scalp acupuncture when drinking 5 ml of water(P <0.01).Experiment 2:1.Based on the comparability of baseline data(P >0.05),Each curative effect index of the experimental group was significantly different before and after treatment(P <0.01),There were significant differences in drinking water test before and after treatment(P <0.01),FOIS grade and SSA score before and after treatment(P <0.05);2.There was significant difference between the two groups in all curative effect indicators after treatment(P<0.05),The experimental group was better than the control group;3.two groups of clinical efficacy comparison,The experimental group cured 15cases(55.55%),7 cases(25.92%),3 cases(11.11%),2 cases(7.40%),Total effective rate 92.59 per cent;There were 9 cases(31.03%),8 cases(27.58%),6 cases(20.68%),6 cases(20.68%),The total effective rate is79.31.Conclusion : 1.this study suggests that there is an immediate effect on the improvement of hyoid bone displacement in patients with dysphagia after stroke.2.In the treatment of dysphagia after stroke,scalp acupuncture synchronous rehabilitation training is more effective than post-head acupuncture rehabilitation training.3.The reason for the better curative effect of scalp acupuncture synchronous rehabilitation training may be the immediate effect of scalp acupuncture to improve hyoid displacement in patients(women)and better synergistic effect with rehabilitation training.
