Diagnostic Value And Significance Of Serum GABA,NE,ET-1 And VEGF Levels In Patients With COPD Complicated With PH

Objective:To investigate the diagnostic value of serum γ-aminobutyric acid(GABA),norepinephrine(NE),endothelin-1(ET-1)and vascular endothelial growth factor(VEGF)in patients with COPD complicated with PH(COPD-PH).Methods:A total of 111 patients with COPD who were admitted to respiratory and critical care department of Shanxi Bethune Hospital from December 2019 to December 2020 were selected and divided into COPD group(PASP≤50mm Hg)and COPD-PH group(PASP>50mm Hg)according to the results of pulmonary artery systolic pressure(PASP)estimated by transthoracic echocardiography(TTE),and lung function and chest CT results were collected.Blood routine,blood biochemical,coagulation function and arterial blood gas analysis were recorded in all patients.Serum levels of γ-aminobutyric acid(GABA),norepinephrine(NE),endothelin-1(ET-1),vascular endothelial growth factor(VEGF)and other cytokines were detected.To compare whether there is statistical difference in the distribution of the above indicators between the two groups;Binary Logistic regression analysis was performed to determine the influencing factors of COPD-PH,with the correlation indicators with statistically different distribution between the two groups as independent variables and whether pulmonary hypertension(PH)was the dependent variable.The ROC curve of serum GABA,NE,ET-1,VEGF and the combined predictors of four factors were analyzed,and the area under the curve(AUC)and the optimal cut-off value were calculated,and the predictive value and clinical significance of the AUC for disease diagnosis were discussed.Results:1.The serum NE and ET-1 in COPD-PH group were significantly higher than those in COPD group,while the serum GABA and VEGF were significantly lower than those in COPD group.2.Binary Logistic regression analysis showed that the risk factors of pulmonary hypertension were serum GABA,NE and VEGF(P<0.05).3.ROC analysis and ROC curve results showed that the AUC of the combined diagnosis of serum NE,ET-1,GABA and VEGF was 0.966(sensitivity was 87.5%,specificity was 93.65%).Conclusion:The AUC of the combined diagnosis of serum NE,ET-1,GABA and VEGF was0.966,the sensitivity was 87.5%,the specificity was 93.65%,and the diagnostic value was higher.
