Objective: To collect medical personnel sharp injury occurred from 2012 to 2018 data and combining the sharp injury questionnaire survey, understand the medical staff sharp injury occurred the status quo, analysis of the characteristics of the sharp injury occurred, influence factors of sharp injury prevention and intervention targeted suggestions for hospital managers.Methods:(1)Literature study.Through searching, collecting and reading a large number of literatures related to sharp instrument injury of medical personnel, the current research status of sharp instrument injury was understood, and the research ideas and methods were provided for this study through literature learning.(2)Questionnaire survey. The questionnaire of sharp instrument injury for medical personnel was designed, and the relevant experts were consulted.A questionnaire survey was conducted on medical personnel who need to use sharp instrument in daily diagnosis and treatment operation in some departments of the hospital.(3)Statistical methods. In this paper, the composition ratio and the number of cases were used for counting data, and Pearson correlation analysis and variance analysis were used to analyze the relationship between the occurrence of sharp instrument injuries and single factors such as gender, professional title and working years.Multivariate Logistic regression analysis was used to analyze the influence of occupational protection knowledge, working environment and other factors on sharp instrument injury of medical staff. Results: Analysis of sharp instrument injury data from 2012 to 2018.(1)The main occurrence of sharp instrument injury among medical personnel: 114 cases(31.84%)of medical instruments and waste,64 cases(17.88%)of operation,59 cases(16.48%)of injection,40 cases(11.17%)of blood collection;(2)The main locations of sharp instrument injuries among medical staff were: 200 cases(55.87%)in wards,70 cases(19.55%)in operating rooms,47 cases(13.13%)in treatment rooms,24 cases(6.7%)in outpatient and emergency departments, and 17 cases(4.75%)in others.(3)There were 109 cases(30.45%)with scalp steel needle,61 cases(17.04%)with syringe and 38 cases(10.61%)with surgical needle.(4)The main exposed site of sharp instrument injury of medical staff was finger,260 cases(72.63%);(5)Among different professions,45 cases(45.45%)of doctors mainly occurred in the operating room;160 cases(66.12%)were mainly nurse in wards.6 cases(35.29%)were mainly in wards.Among the occurrence sites of different professional titles,114 cases(61.29%)of primary professional titles were in wards. The main site of sharp instrument injury was operating room in 31 cases(51.67%).84 cases(75%)were mainly in wards and operating rooms.Among the different professions,52 cases(52.53%)occurred in the course of operation. In nurses,94 cases(38.84%)occurred mainly when dealing with waste and equipment.Among the occurrence times of different professional titles, the primary professional titles were mainly waste treatment and equipment,70 cases(37.63%);The middle and advanced patients were mainly operated,32 cases(56.67%).The difference was statistically significant(P < 0.05).Questionnaire survey of data on sharp injuries among medical staff(1)Gender is related to sharp injury of medical staff(squared =15.48,P<0.05),and the probability of sharp injury in men is greater than that in women;(2)The incidence of sharp injury related to occupational types(squared = 11.63,P < 0.05),the probability of sharp injury compared to nurses and doctors in bigger;(3)The incidence of sharps injuries related to the length of service of the medical staff(squared = 27.101,P < 0.01),the longer working life, the lower the incidence of sharps injuries, vice versa. The incidence of sharp instrument injury among medical personnel with working years ≤5 years was 37.95%;The incidence of sharp instrument injury with 6 to 10 years of working life was 21.99%;The incidence of sharp instrument injury with working years ≥11 years was 12.70%;(4)Medical personnel sharp injury happening with sharp injury protection knowledge(squared = 57.411,P < 0.05),462 subjects, knowledge of various protective medical personnel sharp injury incidence is 14.91%;The incidence rate of sharp instrument injury was 26.88% among medical personnel who had attended the training about sharp instrument injury protection and were familiar with the knowledge of sharp instrument injury protection. The incidence rate of sharp instrument injury was 59.46% among medical personnel who had not participated in occupational protection knowledge training or had knowledge of sharp instrument injury protection. There was statistical significance between the knowledge of sharp instrument injury prevention and the incidence of sharp instrument injury.(5)The occurrence of medical personnel sharp injury related to work environment(squared = 70.44,P < 0.05),in a comfortable work environment with a bright, sharp injury incidence is 10.60%;In the condition of moderate working environment, the incidence of sharp instrument injury was 19.77%.In the condition of poor operating environment, the incidence of sharp instrument injury was 47.22%.In the crowded and disorderly working environment, the incidence rate of sharp instrument injury was 54.08%.The incidence of sharp instrument injury was statistically significant in different operating environments.(6)Multivariate Logistic regression analysis was conducted on the factors influencing the occurrence of sharp instrument injury among medical personnel. It was found that gender, occupation type, professional title, working years, working environment and knowledge of sharp instrument injury prevention were the important factors influencing the occurrence of sharp instrument injury among medical personnel.(7)This study found that the main reasons leading to sharp instrument injury include:violation of operating procedures(36%),unskilled operation(24%),work stress and fatigue(15%),poor working environment(11%),accidental injury of medical staff(8%),and patient’s non-cooperation(6%);(8)The study found that medical staff did not report in accordance with the requirements after the sharp injury happens reason mainly is: the lack of safety consciousness, thinks instruments produce pollution(32%),fear of blame and punishment received supervisor(21%),a stroke of luck, don’t think even stabbed at risk report(19%),the program does not understand(12%),no time to report(9%),reporting program trivial(7%).Conclusion: The survey results show that the high-risk group of medical staff suffering from sharps injuries is mainly nurses. The main characteristics are low professional titles and low working years. The improvement of protection knowledge and the improvement of working environment can effectively reduce the occurrence of sharps injuries.According to the results of the survey, suggestions for the prevention of sharps injuries include: 1.Strengthen the learning and training of sharps injury protection knowledge for medical staff, and improve safety awareness;2.Establish standardized and practical operating specifications;3.Improve sharps injuries The management system provides institutional guarantees for the prevention of sharp object injuries;4.Establish employee electronic health records to improve occupational environmental safety;5.Reasonably allocate human resources to improve the working environment;6.Equip protective safety equipment and promote the use of safety equipment;7.Constructing the occupational safety culture of medical personnel and enhancing the awareness of protection;8.Establishing occupational protection regulations for medical personnel to provide legal protection for the occupational safety of medical personnel.
