Objective:Summarize the clinical characteristics of nasal inverted papilloma,analyze various factors that affect postoperative recurrence,so as to summarize experience,reduce risk factors,and reduce postoperative recurrence rate.Methods:To retrospectively collect the clinical data of 121 patients with nasal inverted papilloma from January 2009 to December 2019 in the the First Hospital of Shanxi Medical University,summarize the clinical characteristics of nasal inverted papilloma,and count the recurrence after surgery Rate,and analyze the relevant factors that affect the patient’s postoperative recurrence.Results:1.Among 121 patients with nasal inverted papilloma,82 were males(67.8%)and 39 were females(32.2%).The sex ratio was about 2.10;the age range was between 28-78 years old,and the average age was about 54 years old.The most common lesions were 117cases(96.7%)involving unilateral and 4 cases(3.3%)on both sides;the tumor occurred in81 cases(66.9%)on the lateral wall of the nasal cavity,followed by 32 cases(26.4%)of the maxillary sinus.),6 cases of ethmoid sinus(10.7%),1 case of frontal sinus(0.8%),and1 case of sphenoid sinus(0.8%).Nasal congestion is the most common main symptom,with 82 cases(67.8%),followed by runny nose in 22 cases(18.2%).There were 29 cases(24%)with ipsilateral nasal polyps.2.There were 20 cases of recurrence after operation,the recurrence rate was 16.5%;14 cases(70%)recurred after 3 years after operation,and 6 cases(30%)recurred within 3years.There were 2 cases of malignant transformation(1.7%).3.The chi-square test was used for univariate analysis,gender(P=0.772),age(P=0.774),course of disease(P=0.474),surgical method(P=0.744)were not statistically significant,and the history of nasal surgery(P =0.018),the number of nasal surgery(P=0.015),tumor site(P=0.002),Krouse staging(P=0.033),pathological type(P=0.014)were statistically significant.4.Logistic binary regression was used to analyze the positive single factors,the number of operations(P=O.015,OR= 2.760,95%CI 1.222-6.237),the tumor occurred in the maxillary sinus(P=0.026,OR=4.402,95 %CI 1.198-16.179),Krouse staging(P=O.046,OR=3.078,95%CI 1.021-9.275),pathological type(P=0.002,OR=3.086,95%CI1.513-6.291)are affecting intranasal Risk factors for recurrence after endoscopy.The ROC curve was drawn according to the variables in the regression model,and the area under the curve was 0.860(P < 0.001,95%CI 0.764-0.956).The prediction model of the ROC curve was statistically significant.The number of surgeries,the occurrence of the tumor in the maxillary sinus,Krouse stage,and the presence or absence of atypical tumor had a sensitivity of 0.65 and a specificity of 0.95 for predicting recurrence.Conclusion:1.Unilateral nasal polyps appearance of the mass should be alert to the possibility of nasal inverted papilloma,preoperative and intraoperative pathological examination as far as possible in order to select a reasonable surgical method.Long-term follow-up is necessary for patients with inverted papilloma.2.There was no significant correlation between gender,age,course of disease and surgical method and inverted papilloma.4.The number of operations,the occurrence of the tumor in the maxillary sinus,Krouse stage,and the presence of atypical hyperplasia are the risk factors for postoperative recurrence of inverted papilloma.The number of surgeries was once,the tumor originated from non-maxillary sinus,Krouse stage T1,and the pathology was not accompanied by atypical hyperplasia.The probability of tumor recurrence excluded was 95%.
