Predictive Factors Associated With Interstitial Lung Disease In Patients With Dermatomyositis Or Polymyositis:A Meta-analysis

Objective:To identify the predictive factors of interstitial pulmonary disease(ILD)associated with dermatomyositis/polymyositis(DM/PM),and to provide help for the early diagnosis and treatment of DM/PM associated ILD in clinical work.Methods:All literatures about DM/PM ILD studies published in CNKI,CBM,VIP,Wanfang,PubMed,Embase,Web of Science and Cochrane Library in Chinese and English from 1998 to 2021 were searched by computer.Two researchers independently completed the preliminary work of the meta-analysis.On this basis,the software was used for meta-analysis and the conclusions were drawn.Results:Finally,46 studies were analyzed.Among these patients,1,951 cases were combined with ILD as case group;2975 patients without ILD were regarded as the control group.The results of the meta-analysis were as follows: the OR value and 95%CI of female sex combination were 0.8(0.74,0.98),P=0.02;The combined OR value of Gottron Eigenvalue and 95%CI were 0.82(0.57,1.17),P=0.28;The combined OR and95%CI of Heliotrope rash were 1.45(0.96,2.18),P=0.08;The OR value and 95%CI of arthritis/arthralgia were 2.81(2.37,3.32),P<0.00001;The combined OR and 95%CI of Raynaud’s phenomenon were 2.3(1.72,3.07),P<0.00001.The OR value and 95%CI of dysphagia were 0.96(0.71,1.29),P=0.78;The OR value and 95%CI of malignant tumor were-0.07(-0.12,-0.01),P=0.01;Fever combined OR value and 95%CI were 3.76(2.81,5.01),P < 0.00001;ANA positive combined OR value and 95%CI were 1.29(0.96,1.74),P=0.09;The combined OR value and 95%CI of anti-jo-1 antibody were 4.01(3.06,5.25),P<0.00001。Conclusion:female,joint symptoms,Raynold’s sign,fever,malignant tumor and anti-JO-1antibody were the predictors of DM/PM-ILD.Gottron sign,Heliotrope rash,dysphagia,positive ANA and ILD were not significantly correlated with DM/PM.
