| Objective:To review and analyze the clinical characteristics of dult polymyositis(PM)and adult dermatomyositis(DM),and to analyze independent risk factors for adult PM/DM with interstitial lung disease(I LD).Method: Reviewed and analyzed 252 PM/DM adult patients(52 PM and 200 DM)who were hospitalized and diagnosed in the department of rheumatic immunity in the second hospital of Hebei medical university in january 2015~october 2019.Compared and analyzed the character istics and differences of clinical data of PM and DM patients,and analyzed independent risk factors for concurrent ILD and DM.Results:PM and DM were mostly female.The peak age for PM and DM was 41~60 years.The first symptom of PM had the highest probability of muscle weakness,the incidence of rash was highest in the first symptom of DM.The highest rates of PM and DM muscle damage were found in limb weakness.DM characteristic skin changes were most common in sunny rash,followed by Gottron sign,shawl sign,anterior cervical V rash.PM patients had higher risk of upper limb weakness and lower limb weakness than DM patients,PM patients had lower risk of joint pain than DM patients.Myoenzymes profile in PM patients was most common with elevated CK,followed by the increase of MYO、LDH,The increase of LDH was most common in DM patients,followed by elevated HBDH.Patients with PM had higher rates of CK,CKMB,AST,ALT and MYO than patients with DM.The probability of myogenic damage and neurogenic damage in PM and DM patients was approximately the same.PM and DM patients are prone to concurrent ILD.SSA antibody positive,Jo-1 antibody positive,Ro52 antibody positive,and muscle soreness were independent risk factors for PM complicated ILD.Anti-SSA antibody positive,anti-antibody positive,anti-joint pain were independent risk factors for DM complicated ILD.Conclusion:1.PM and DM are mostly female,with a peak age of 41~60 years.PM patients are more prone to limb muscle weakness than DM patients.DM patients are more likely to develop joint pain than PM patients.PM patients are more prone to CK、CKMB、AST、ALT、MYO elevation than DM patients.2.PM and DM patients are prone to concurrent ILD.SSA antibody positive,Jo-1 antibody positive,Ro52 antibody positive,and muscle soreness are independent risk factors for PM complicated ILD.Anti-SSA antibody positive,anti-antibody positive,anti-joint pain are independent risk factors for DM complicated ILD. |
