| Objective:This topic mainly studies the position changes of the intraocular lens after phacoemulsification and intraocular lens implantation in patients with high myopia and cataract,including the tilt and decentration of the intraocular lens.To study the change of the effective lens position and find the relationship between effective lens position and the tilt and decentration of the intraocular lens,so as to provide some theoretical guidance for improving the visual quality of such patients after surgery.Methods:Collected from October 2019 to June 2020 in Anhui Provincial Hospital Affiliated to Anhui Medical University,21 patients and 32 eyes were diagnosed with high myopia combined with cataract and underwent phacoemulsification combined with intraocular lens implantation in our hospital According to the difference of intraocular lens(IOL)implantation,it can be divided into two groups: HD group(with SOFTEC HD lens implantation)15 eyes,including 9 males and 6 females,10 left eyes and 5 right eyes Only;In AO group,17 eyes were implanted with Akreos-AO crystal,including 7 males,10 females,9 left eyes and 8 right eyes.The uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA),effective lens position(ELP),IOL tilt and IOL decentration were recorded on the first day,1 week,1 month and 3 months after surgery.Compare the two groups,whether there is statistical differences within the group.At the same time,whether there is correlation between the effective lens position at different time points and the inclination and eccentricity of the intraocular lens was compared.Results:There were no significant differences in UCVA,BCVA,ELP,IOL tilt,and IOL decentration at each time point between the two groups(P>0.05).Comparison of different time points within the group: HD group(ELP F=19.706,P=0.001,IOL tilt F=15.901,P=0.001,IOL decentration F=19.907,P=0.001,P < 0.01),AO group(ELP F=14.279,P=0.001,IOL tilt F=69.576,P=0.001,IOL decentration F=40.263,P=0.001,P< 0.01).Indicating the two groups,ELP,IOL tilt,and IOL decentration have statistical significance at four different time points.There is no correlation between ELP and IOL tilt at each time point(r=0.244,P=0.179 1 day after operation,r=0.15,P=0.414 1 week after operation,r=0.32,P =0.074 1 month after operation,after 3 months r=0.273,P=0.13,P >0.05),ELP and IOL decentration were positively correlated at each time point(1 day after operation r=0.534,P=0.002,surgery One week later,r=0.552,P=0.001,1 month after operation r=0.647,P=0.001,3 months after operation r=0.553,P=0.001,P <0.01).Conclusion:In patients with high myopia combined with cataract,ELP moves significantly after surgery.The IOL moved forward slightly within one to one week,and moved later within one week to one month after surgery.One month after operation,ELP is stable.At the same time,it was found that the postoperative ELP was positively correlated with the decentration of the intraocular lens,but not significantly related to the tilt of the intraocular lens. |
