Objective: To observe the short-term decentration and tilt after intraocular lens implantation for eyes with long axial length,and to explore the factors influencing the decentration and tilt after intraocular lens implantation.Methods: A prospective analysis was made on 92 cases of phacoemulsification and IOL capsular implantation.Three groups were divided by the length of axial: the axial24-26 mm group with 30 eyes;the axial 26-28 mm group of 28 eyes;the axial greater than 28 mm group 34 eyes.All patients underwent phacoemulsification combined with IOL implantation by the same surgeon,and the decentration and tilt,kappa Angle and alpha Angle,tilt(4mm/6mm)of the IOL after implantation were followed up 1 week,1month and 3 months after surgery.SPSS24.0 software made statistical analysis of the data.The data in this study were used for the man-whitney test.Decentration and tilt,kappa Angle and alpha Angle,tilt(4/6mm)selected any two for spearman correlation analysisResults: The decentration and tilt of the three groups in each postoperative period(1week /1 month /3 months)increased with the increase of the eye axial.Axial 24-26 mm group and axis 26-28 mm group: there were statistically significant differences in tilt(4/6mm),decentration and tilt between the two groups 1 week and 1 month after surgery(p<0.05).There were statistically significant differences in tilt(4/6mm),decentration and tilt between 1 week after surgery and 3 months after surgery(p<0.05).In the axial24-26 mm group,the difference of decentration between 1 month after surgery and 3months after surgery was statistically significant(p<0.05).In the axial greater than 28 mm group:the difference of tilt(4mm),decentration and tilt between 1 week and 1 month after the operation was statistically significant(p<0.05).There were statistically significant differences in tilt(4mm),decentration and tilt between 1 week and 3 months after surgery(p<0.05).The difference of decentration between 1 month and 3 months after the operation was statistically significant(p<0.05).Kappa Angle was weakly positively correlated with decentration(p=0.014).Decentration,tilt and tilt(4mm)were weakly positively correlated(p=0.003/p=0.005).Decentration,tilt and tilt(6mm)show a weak positive correlation(p=0.006/p=0.017).Conclusion: The longer the axial,the more decentration and tilt will increase.The indicators related to decentration and tilt of the eye of different lengths were different between 1 week and 1 month after the operation,1 week after the operation and 3 months after the operation.The anterior capsule began to shrink one month after the operation,the space for IOL to move decreased,and the position was generally stable.OPD Scan-Ⅲ aberration instrument with Pentacam three-dimensional anterior segment analyzer based on the analysis of IOL decentration and tilt has certain relevance.In addition to the previous preoperative guidance of intraocular lens selection,kappa Angle has a certain correlation with the stability index of intraocular lens,which is another discovery.
