| BackgroundAcute myocardial infarction(AMI)is the leading cause of death in cardiovascular disease.Rapid diagnosis of AMI and its slow progression are critical.With the development of bioinformatics,more and more studies believe that micro RNA may be a new biomarker for the diagnosis and prognosis of AMI.However,it is still unclear whether micro RNA can be used as biomarkers for the diagnosis of AMI and its potential mechanism.Previous studies found that let-7c-5p was co-expressed with circ RNAs and regulated the development of AMI.Therefore,this study screened the differentially expressed let-7c-5p through micro RNAs expression profile analysis in patients with AMI,verified its level changes in AMI,and further analyzed its clinical significance in the progression of AMI.ObjectiveTo study the expression level of let-7c-5p in the circulation of patients with AMI,and to explore the clinical significance of let-7c-5p in the progression of AMI,so as to provide reference evidence for the exploration of biomarkers of AMI.MethodsAccording to the diagnostic criteria of AMI and unstable angina pectoris,a total of152 blood samples were screened from the Engineering Research Center of Clinical Data and Samples of Cardiovascular Diseases in Henan Province from November 2016 to January 2021,and they were divided into AMI group and control group.Among them,15cases(5 cases in each group)were analyzed by micro RNA expression microarray,and differentially expressed micro RNA were screened to further verify the results of micro RNA expression microarray analysis.Among the other 137 patients,AMI group(n =47),unstable angina pectoris group(n = 42)and control group(n =48),blood samples were collected from patients in each group.Total RNA was extracted from peripheral blood and c DNA was synthesized by reverse transcription.Circulating let-7c-5p levels were detected by real-time reverse transcription polymerase chain reaction.Cardiac markers in plasma were detected by automatic biochemical analyzer.Spearman correlation analysis was performed to further analyze the correlation between different biochemical indexes of heart function and let-7c-5p.According to the diagnostic criteria of AMI,137 patients were divided into AMI group and non-AMI group.Univariate and multivariate factors affecting AMI were analyzed to explore the relationship between let-7c-5p and AMI.The area under the receiver operating characteristic curve was used to determine the truncation value of let-7c-5p,and the predictive value of let-7c-5p for the diagnosis of AMI was analyzed.Results1.Compared with unstable angina pectoris group and control group,the microarray analysis results of micro RNA expression profile showed that let-7c-5p,miR-6832-5p,miR-1973,miR-4485-3p,miR-8063,miR-3912-5p,miR-193a-5p miR-8063 and miR-6793-5p were highly expressedin AMI patients,miR-6749-5p,miR-328-5p,miR-6716-3p and miR-1202 were lower expression in AMI patients.2.Compared with unstable angina pectoris group and control group,the levels of circulating let-7c-5p in AMI patients were significantly increased(P < 0.001).3.Spearman correlation analysis showed that let-7c-5p was positively correlated with MYO、CK-MB、c Tn I,suggesting a close correlation with myocardial injury.Let-7c-5p is negatively correlated with LVEF and LVFS,which may be correlated with cardiac function injury to some extent.4.Multivariate binary Logistic regression analysis further studied the most significant factors affecting AMI,and the results showed that let-7c-5p was significantly correlated with AMI.Every 1μg/μL increase of let-7c-5p,The probability of AMI was 1.009 times lower by 1μg/μL than that of let-7c-5p(95%CI:1.002-1.016).5.The area under the curve of let-7c-5p for the diagnosis of AMI was 0.900,the sensitivity was 91.5% at the threshold of 60.962 μg/μL,and the specificity was 83.3%,indicating that let-7c-5p provides parameter data for analysis of AMI biomarkers.ConclusionThe up-regulated expression of circulating let-7c-5p in patients with AMI is correlated with myocardial damage to a certain extent,and let-7c-5p is expected to provide parameter data for the analysis of AMI biomarkers. |
