Clinical Analysis Of Etiology Of Ascites In Children

Objective:Analyze the etiology and clinical characteristics of ascites in children,so as to provide theoretical basis for the diagnosis and treatment of ascites in children,and reduce missed diagnosis and misdiagnosis.Methods:Through the scientific research platform of our hospital,the clinical data of 165 children with ascites diagnosed in hospital from January 1,2010 to December31,2019 were collected,and their basic information,laboratory data and etiological diagnosis were retrospectively analyzed.Results:There were 165 cases of ascites in children,including 100 males and 65 females.The onset age ranged from 1 month to 14 years old,with an average age of(5.67 ±4.28)years.The causes of ascites in children were surgical acute abdomen in 39 cases(23.6%),infectious diseases in 39 cases(23.6%),neoplastic diseases in 27 cases(16.4%),hepatogenic diseases in 18 cases(10.9%),pancreatitis in 10 cases(6.1%),cardiogenic diseases in 8 cases(4.8%),rheumatic immune diseases in 6 cases(3.6%),nephrogenic diseases in 5 cases(3.0%).Other causes included 5 cases of eosinophilic gastroenteritis(3.0%),3 cases of urticaria(1.8%),2 cases of chylous abdomen(1.2%),1 case of ulcerative colitis(0.6%),1 case of primary intestinal lymphangiectasis(0.6%)and 1 case of ureteral obstruction(0.6%).Among them,33cases(20%)were infants(28 days～1 year old),24 cases(14.5%)were infants(1～3years old),30 cases(18.2%)were preschool(3～6 years old),41 cases(24.8%)were school age(6～10 years old),37 cases(22.4%)were puberty(10～14 years old).The main causes of ascites in children of different age groups:12 cases of acute abdomen in infant group,compared with other age groups,there was no significant difference(P>0.05),9 cases of hepatogenic diseases,compared with other age groups,the difference was statistically significant(P<0.05);10 cases of tumor diseases in infant group,10 cases of tumor diseases in preschool group,compared with other age groups,the difference was statistically significant(P<0.05)There were 14 cases of infectious diseases in the school age group and 7 cases in the adolescent group,and there was no significant difference compared with other age groups(P>0.05).There were 71 cases(43.0%)with grade 1 ascites,75 cases(45.5%)with grade 2 ascites and 19 cases(11.5%)with grade 3 ascites in children.There was no significant difference in different ascites grades among the etiological groups(P>0.05).Conclusion:The etiology of ascites in children is complex.The common diseases are surgical acute abdomen,infectious diseases,tumor diseases and hepatogenic diseases.Hepatogenic diseases are the main cause of ascites in infants.Biliary atresia is the most common cause of cirrhosis in children.The main causes of ascites in children and preschool children are tumor diseases,malignant tumors are common,lymphoma and leukemia are the main causes;tuberculosis infection is the primary cause of ascites in children caused by infectious diseases.Grade 1 and 2ascites are more common in children.
